声辐射力脉冲弹性成像技术评估精索静脉曲张严重程度研究

Evaluation of varicocele severity by acoustic radiation force impulse

目的探讨应用声辐射力脉冲弹性成像技术(ARFI)评估精索静脉曲张的严重程度。方法选取2019年3—11月在北部战区总医院军人门诊行睾丸及精索静脉检查的60例官兵为研究对象。其中,30例患者有症状且进行手术设为观察组,其余30例无症状的健康官兵设为健康组。所有受检者均行常规超声检查及剪切波弹性成像。比较两组研究对象的剪切波速度(Vs),分析睾丸组织Vs值与精索静脉严重程度的相关性。结果观察组的左、右侧睾丸Vs值均低于健康组,组间比较,差异均有统计学意义(P<0.05)。观察组30例精索静脉曲张患者根据临床分类分为轻度静脉曲张5例、中度静脉曲张15例、重度静脉曲张10例,静脉曲张越严重,Vs值越低。相关性分析显示,左侧睾丸组织Vs值与精索静脉曲张严重程度呈负相关(r=-0.645,P<0.05)。结论ARFI可有效评估精索静脉曲张的严重程度,为临床治疗提供可靠的诊断依据。

Objective To evaluate the severity of varicocele using acoustic radiation force impulse(ARFI).Methods A retrospective study was performed on 60 soldiers who underwent testicular and spermatic vein examinations from March to November 2019.Among them,30 patients with symptoms underwent surgery were divided into the observation group,and the other 30 healthy soldiers without symptoms were divided into the health group.All subjects underwent conventional ultrasound and shear wave elastography.The shear wave velocity(Vs)was compared between the two groups,and the relationship between Vs and the severity of spermatic vein was analyzed.Results The left and right testis Vs of the observation group were lower than those of the healthy group,and the difference was statistically significant(P<0.05).According to clinical classification,30 patients in the observation group were divided into 5 patients with mild varicose veins,15 patients with moderate varicose veins and 10 patients with severe varicose veins.The more severe the varicose vein,the lower the Vs was.Correlation analysis showed that the Vs value of left testicular tissue was negatively correlated with the severity of Varicocele(r=-0.645,P<0.05).Conclusion ARFI can effectively evaluate the severity of varicocele and provide reliable diagnostic basis for clinical treatment.
