从《素问·热论》宣解通透观探讨新型冠状病毒肺炎的治疗

Exploration on the Treatment of COVID-19 from Dispersion and Unblocking in Discussion of Heat of Basic Questions

基于《素问·热论》对温热病邪六经传变规律的论述,阐释新型冠状病毒肺炎(新冠肺炎)的传播途径、发病特点、临床症状、潜伏期、自愈特点及最佳治疗时间。根据"其未满三日者,可汗而已;其满三日者,可泄而已"的治疗原则,把新冠肺炎分为邪在三阳、邪困三阴三阳之间、邪在三阴3个不同的病程阶段。提出邪在三阳,疏表宣解求汗;邪困三阴三阳之间,半里半表,则宣解通透、交通表里玄府,内外分消,以防营卫交并,浊毒内恋膜原;邪在三阴,热壅在中,里气不通,阳邪不能出,阴邪不能下,则通里透表,以防营卫雩重,浊毒内陷。逐邪时应并兼养正,复其脏脉,根据不同的临床阶段和临床表现辨证选用宣解通透之法。

According to the contents on the law of transmission and change in the six channels of warm disease from Discussion of Heat of Basic Questions,the transmission route,onset characteristics,clinical symptoms,incubation period,self-healing characteristics and the best treatment time of COVID-19 were discussed. According to the principle of treatment "the one less than three days could be treated with diaphoresis and more than three days could be treated with purgation",COVID-19 could be divided into 3 stages:evils in three yang,evils trapped between three yin and three yang,and evils in three yin. For evils in the three yang stage,patients should be treated with relieving the exterior and diaphoresis. If the evils are trapped between three yin and three yang,attention should be paid to dredging the inside and outside and the communication of the internal and external organs in treating the disease,so that the evil qi can spread from the inside to the outside through the pores,and can be treated and relieved both inside and outside of the body. At the same time,it is necessary to prevent the simultaneous occurrence of Ying disease and Wei disease,and prevent the retention of turbid toxin in Moyuan(the parts that evils can hide).For evils in the three yin,the toxic heat is stagnant in the body,the inside qi is blocked,the Yang evil can not be dispelled,and the yin evil can not descend. In treating it,the body qi should be made permeable,and the evil qi be dispelled and descend. At the same time,it is necessary to prevent the aggravation of Ying disease and Wei disease,and prevent the invasion of turbid toxin into the viscera. While dispelling evil qi,we should try to maintain vital qi and restore the functions of viscera and vessels. According to clinical manifestations in different stages,the method of dispersion and unblocking should be applied.
