摘要
目的探究人乳头瘤病毒(Human papilloma virus,HPV)感染宫颈病变患者中辅助性T细胞17(T helper cell 17,Th17)、血清白细胞介素-2(Interleukin-2,IL-2)、白细胞介素-10(Interleukin-10,IL-10)、E2F转录因子1(E2F transcription factor 1,E2F-1)水平变化及其临床意义。方法选取2017年5月-2019年5月在三亚市中医院妇科门诊进行宫颈活检的100例患者作为研究对象,包括慢性宫颈炎(Chronic cervicitis,CCS)组28例,宫颈癌(Cervical cancer,CC)组21例,宫颈上皮内瘤变(Cervical intraepithelial neoplasias,CIN)组51例,包括CIN I级16例,II级组18例,III级组17例。检测并比较各组Th17细胞、IL-10、IL-2及E2F-1蛋白表达情况。结果CCS组Th17细胞为(1.19±0.21)%,低于CIN组及CC组,CIN组低于CC组(P<0.05);CCS组IL-2水平为(47.64±4.04)pg/ml,高于CIN组及CC组,CIN组高于CC组(P<0.05);CCS组IL-10水平为(26.42±7.58)pg/ml,低于CIN组及CC组,CIN组低于CC组(P<0.05)。CIN I组Th17细胞为(1.06±0.17)%,低于CINⅡ组及CINⅢ组,CINⅡ组低于CINⅢ组(P<0.05);CIN I组IL-2水平为(46.32±3.14)pg/ml,高于CINⅡ组及CINⅢ组,CINⅡ组高于CINⅢ组(P<0.05);CIN I组IL-10水平为(24.96±6.05)pg/ml,低于CINⅡ组及CINⅢ组,CINⅡ组低于CINⅢ组(P<0.05)。CCS组、CIN I组、CINⅡ组、CINⅢ组、CC组E2F-1蛋白阳性表达率分别为14.29%、31.25%、33.33%、41.18%、85.71%,CC组E2F-1蛋白阳性表达率均高于其他各组,CIN各组E2F-1蛋白阳性表达率均高于CCS组(P<0.05)。结论HPV感染宫颈癌病变患者随着疾病的进一步深入变化,体内Th17水平和IL-10、E2F1水平不断升高,IL-2水平不断降低,可能与高危型HPV感染的宫颈癌患者免疫调节失衡有关,这种变化随着病情的发展愈加明显。
OBJECTIVE To investigate the changes and clinical significance of T helper cell 17(Th17),serum interleukin-2(IL-2),interleukin-10(IL-10)and E2 F transcription factor-1(E2 F-1)in patients with cervical lesions infected by human papillomavirus(HPV).METHODS One huandred patients with cervical biopsy in the gynecological clinic of Sanya Hospital of Traditional Chinese Medicine hospital between May 2017 and May 2019 were recruited.They were divided into the chronic cervicitis(CCS)group with 28 patients,in and with the cervical cancer(CC)group with 21 patients and the cervical intraepithelial neoplasia(CIN)group with 51 patients.All the subjects included 16 cases with CIN I category,18 cases with CIN II categoryand the 17 cases with CIN III category.The number of Th17 cells and expression levels of IL-10,IL-2 and E2 F-1 proteins in each group were detected and compared.RESULTS Th17 cells in the CCS group were(1.19±0.21)%,which was significantly lower than that in the CIN group and CC group;Th17 cells in the CIN group was significantly lower than that in the CC group(P<0.05).The level of IL-2 in the CCS group was(47.64±4.04)pg/ml,which was significantly higher than that in the CIN and CC groups;The level of IL-10 in the CIN group was significantly higher than that in the CC group(P<0.05).The level of IL-10 in the CCS group was(26.42±7.58)pg/ml,significantly lower than that in the CIN group and CC group;and the index in CIN group was lower than that in the CC group(P<0.05).The number of Th17 cells in the CIN I group was(1.06±0.17)%,significantly lower than that in CIN II group and CINⅢgroup;and the corresponding index in the CINⅡgroup was significantly lower than that in the CINⅢgroup(P<0.05).The level of IL-2 in the CIN I group was(46.32±3.14)pg/ml,which was significantly higher than that in the CINⅡgroup and CINⅢgroup.The level of IL-10 in the CINⅡgroup was higher than that in the CINⅢgroup(P<0.05).The level of IL-10 in the CIN I group was(24.96±6.05)pg/ml,significantly lower than in the CINⅡgroup and CINⅢgroup;and the level of IL-2 in the CINⅡgroup was significantly lower than that in the CINⅢgroup(P<0.05).The positive expression rates of E2 F-1 protein in the CCS group,CIN I group,CINI group,CIN I group and CC group were 14.29%,31.25%,33.33%,41.18%and 85.71%respectively.The positive expression rate of E2 F-1 protein in the CC group was significantly higher than that in the other groups;and the positive expression rate of E2 F-1 protein in the CIN group was significantly higher than that in the CCS group(P<0.05).CONCLUSION With the progression of the disease,the levels of Th17,IL-10 and E2 F1 in patients with cervical cancer infected by HPV were continuously increasing,whereas the level of IL-2 is continuously decreasing,which may be related to the imbalance of immune regulation in patients with cervical cancer infected by high-risk HPV.This change became more and more obvious with the development of the disease.
