大承气汤治疗重症急性胰腺炎临床疗效的Meta分析

Meta-analysis of Dachengqi Decoction on Severe Acute Pancreatitis

目的对大承气汤治疗重症急性胰腺炎的临床疗效进行系统评价。方法Pubmed、CBM、中国期刊全文数据库、维普中文科技期刊、万方数据库、中国生物医学数据库中公开发表的学术论文,所有中英文数据库的检索时限均从建库至2019年12月31日,按纳入排除标准进行文献筛选,评价入选文献的质量并提取相关资料,用RevMan5.3软件对治疗总有效率、腹痛缓解时间、排气恢复时间和住院时间进行Meta分析。结果共纳入12篇临床研究文献,病例总数975例。Meta分析结果显示:治疗组临床有效率优于对照组[OR=6.74,95%CI(3.89,11.69),P<0.00001];治疗组能够显著缩短腹痛持续时间[MD=-1.85,95%CI(-2.49,-1.20),P<0.00001]、排气恢复时间[MD=-2.21,95%CI(-3.31,-1.11),P<0.00001]、住院时间[MD=-7.65,95%CI(-10.77,-4.53),P<0.00001]。结论大承气汤能够显著改善重症急性胰腺炎患者腹痛的症状,加快排气恢复,缩短疾病的病程,减少住院时间,提高疗效。

Objective:To systematically evaluate the clinical efficacy of Dachengqi Decoction on severe acute pancreatitis.Methods:Published academic papers in pubmed,CBM,Chinese Journal Full-text Database,VIP,Wanfang Database and China Biomedical Database were searched.The retrieval time limit of all Chinese and English databases was from the establishment of the database to December 31,2019.Literature screening was performed according to the inclusion and exclusion criteria;the quality of the selected literatures was evaluated,and relevant data were extracted.RevMan 5.3 software was used to perform a meta-analysis of the total treatment effectiveness,abdominal pain relief time,exhaust recovery time and the length of hospital stay.Results:A total of 12 clinical research literature was included,including 975 cases.Meta analysis results show that the clinical effectiveness of the treatment group was better than that of the control group[OR=6.74,95%CI(3.89,11.69),P<0.00001];the treatment group could significantly reduce the duration of abdominal pain[MD=-1.85,95%CI(-2.49,-1.20),P<0.00001],exhaust recovery time[MD=-2.21,95%CI(-3.31,-1.11),P<0.00001],and hospital stay[MD=-7.65,95%CI(-10.77,-4.53),P<0.00001].Conclusion:Dachengqi Decoction can significantly improve the symptoms of abdominal pain in patients with severe acute pancreatitis,accelerate the recovery of exhaust,shorten the course of disease,reduce the length of hospital stay,and improve the curative effect.
