摘要
目的探讨幽门螺杆菌感染的慢性胃炎患者紧密连接蛋白-1(Zonula occluden-1,ZO-1)、Fas跨膜蛋白(Factor associated suicide,FAS)、矮小相关转录因子3(Runt-related transcription factor 3,Runx3)蛋白表达变化与组织病理特征的关系。方法采集2018年1月-2019年1月九江市第三人民医院消化内科95例幽门螺杆菌感染的经内镜检查确诊的慢性胃炎患者胃黏膜表面的病理样本,分为慢性萎缩性胃炎(Chronic atrophic gastritis,CAG)组25例、慢性非萎缩性胃炎(Chronic superficial gastritis,CSG)组28例、肠化生(Intestinal metaplasia,IM)组23例、上皮内瘤变(Intraepithelial neoplasia,IN)组19例,并以无幽门螺杆菌感染的慢性胃炎患者为对照组。免疫组化分析上皮细胞ZO-1、Runx3蛋白表达变化。流式细胞术分析胃上皮细胞中的Fas受体表达水平。结果与未感染Hp的对照组相比,幽门螺杆菌感染的慢性胃炎患者ZO-1蛋白降低,IM、IN组低于其他病理类型(P<0.001);Runx3蛋白水平降低(P<0.001);Fas蛋白水平升高(P<0.001),CAG、IM、IN组Fas蛋白水平高于CSG组(P<0.001)。ZO-1蛋白水平与EDS评分呈负相关性(r=-0.572,P=0.0278),Fas蛋白表达与EDS评分呈正相关性(r=0.510,P=0.0343)。结论幽门螺杆菌感染的慢性胃炎患者胃上皮细胞ZO-1、Fas、Runx3蛋白表达变化与不同病理类型有关,ZO-1、Fas蛋白与胃黏膜损伤程度密切相关。
OBJECTIVE To investigate the association between the expression of Zonula occluden-1(ZO-1), factor associated suicide(Fas), Runt-related transcription factor 3(Runx3) and histopathological characteristics in chronic gastritis patients infected with Helicobacter pylori. METHODS Pathological samples of 95 H. pylori infection patients with chronic gastritis diagnosed by endoscopy were collected from Jan. 2018 to Jan. 2019 in gastrology department of the Third People’s Hospital of Jiujiang and divided into four groups: CAG(n=25), CSG(n=28), IM(n=23) and IN(n=19). Patients diagnosed with no H. pylori infection were enrolled as the control group. Immunohistochemical analysis was performed to measure ZO-1, Runx3 protein expression changes in epithelial cells. Fas receptor expression in gastric epithelial cells was analyzed by flow cytometry. RESULTS Compared to the control group who were not infected with H. pylori, ZO-1 protein was significantly decreased in the chronic gastritis patients infected with H. pylori;levels of ZO-1 protein in the IM and IN groups were significantly lower than that in other pathological types(P<0.001). The level of Runx3 protein was significantly decreased compared to that in the control group(P<0.001). Fas protein level was higher in CAG, IM and IN group than that in the CSG group(P<0.001). ZO-1 protein level was negatively correlated with EDS score(r=-0.572, P=0.0278), whereas Fas protein expression was positively correlated with EDS score(r=0.510, P=0.0343). CONCLUSION The expression of ZO-1, Fas and Runx3 in gastric epithelial cells of patients with chronic gastritis infected by H. pylori was related to the different pathological types, and ZO-1 and Fas proteins were closely related to the degree of gastric mucosal injury.
