喉癌患者术后复发与COX-2表达的关系研究

Relationship between postoperative recurrence and COX-2 expression in patients with laryngeal cancer

目的探讨喉癌组织中环氧化酶-2(COX-2)蛋白表达与肿瘤术后复发的关系。方法选取2013年1月-2015年8月我院实施手术治疗的喉癌患者81例,对患者进行3年随访,根据是否发生局部复发分为复发组27例和未复发组54例,采用免疫组化染色检测两组手术后病理学标本中COX-2蛋白表达,对比两组的一般临床资料,采用Logistic多因素方法分析COX-2对喉癌术后复发的影响。结果经秩和检验分析,复发组患者手术后喉癌组织中的COX-2蛋白表达强度显著高于未复发组患者,差异具有统计学意义(P<0.05);复发组患者的低分化占比、发生淋巴结转移率均高于未复发组的患者,差异具有统计学意义(P<0.05);复发组和未复发组患者的TNM分期、肿瘤位置分布比较,差异无统计学意义(P>0.05);采用Logistic回归分析结果显示分化程度越低、发生淋巴结转移、COX-2阳性表达是喉癌患者术后复发的独立危险因素(P<0.05)。结论喉癌患者术后复发的危险因素有多种,COX-2蛋白表达强度增强是危险因素之一。

Objective To investigate the relationship between cyclooxygenase-2(COX-2)protein expression and tumor recurrence in laryngeal carcinoma.Methods Eighty-one patients with laryngeal cancer who underwent surgery in our hospital from January 2013 to August 2015 were enrolled.The patients were followed up for 3 years.According to whether local recurrence occurred,27 patients were relapsed and 54 patients were not relapsed.The expression of COX-2 protein in pathological specimens of the two groups was detected by staining.The general clinical data of the two groups were compared.Logistic multivariate analysis was used to analyze the effect of COX-2 on postoperative recurrence of laryngeal cancer.Results According to the rank sum test,the intensity of COX-2 protein expression in laryngeal cancer tissues of patients with recurrent group was significantly higher than that of patients without recurrence(P<0.05).The proportion of patients with relapse group was poorly differentiated.The rate of lymph node metastasis was higher than that in the non-recurrent group(P<0.05).There was no significant difference in TNM stage and tumor location between the relapsed group and the non-recurrent group(P>0.05).Logistic regression analysis showed that the lower the degree of differentiation,lymph node metastasis and COX-2 positive expression were independent risk factors for postoperative recurrence of laryngeal cancer patients(P<0.05).Conclusion There are many risk factors for postoperative recurrence in patients with laryngeal cancer,and the increased intensity of COX-2 protein expression is one of the risk factors.
