动静态结合训练对脑卒中偏瘫患者平衡功能、步行能力及生活质量的影响分析 被引量:10

摘要 目的:脑卒中后偏瘫一直是临床关注重点,脑卒中后偏瘫康复方案较多,且随着医学技术的快速发展,其康复方案日益增多,临床应用时争议较大,该研究旨在分析脑卒中偏瘫患者应用动静态结合训练干预后效果。方法:将我院2018年1月至2019年1月期间收治的107例脑卒中偏瘫患者分为对照组和观察组,对照组53例患者应用常规肢体康复训练,观察组54例患者增加视觉运动想象(visual imagery,VI)训练,对比两组患者干预前后平衡功能、步行能力、生活质量变化。结果:观察组患者Brunel评分(43.20±3.62)分高于对照组(36.39±2.98)分(t=10.6132,P <0.001);观察组步行能力步3级低于对照组(χ2=4.2410,P <0.05),5级高于对照组(χ2=4.2322,P <0.05);观察组SF-36评分(163.35±11.21)分均高于对照组(146.65±10.01)分(t=8.1230,P=0.0000)。结论:动静态结合训练能够有效提升脑卒中偏瘫患者步行能力及平衡功能,改善生活质量。 Objective: Post-stroke hemiplegia(PSH) has been the focus of clinical attention for a long time.There are various regimens for rehabilitation in PSH. With the rapid development of medical technologies, the number of rehabilitation regimens is increasing, which results in numerous controversies in the clinical application. The purpose of this study is to analyze the outcome of patients with PSH after intervention with dynamic and static training. Methods: A total of 107 patients with PSH who were admitted to our hospital from January 2018 to January2019 were divided control group and observation group. Fifty-three patients in the control group adopted conventional limb rehabilitation training, and 54 patients in the observation group adopted visual imagery training in addition to the training for the control group. The two groups were compared for changes in balance function, walking ability,and quality of life after intervention. Results: The Brunel score of the observation group was significantly higher than that of the control group(43.20 ± 3.62 vs 36.39 ± 2.98, t = 10.6132, P < 0.001);compared with the control group, the observation group had reduced walking ability in terms of grade 3 score(c2= 4.2410, P < 0.05), but had improved walking ability in terms of grade 5 score(c2= 4.2322, P < 0.05);the observation group had significantly higher 36-Item Short-Form Health Survey scores than the control group(163.35±11.21 vs 146.65±10.01, t =8.1230, P = 0.0000). Conclusion: Dynamic and static training can effectively improve the walking ability, balance function, and quality of life of patients with PSH.
