社区健康服务中心开展慢性病长期处方的现状及可行性研究

A Study on the Status and Feasibility of Long-term Prescription for Chronic Diseases in Community Health Service Centers

目的探索慢病长期处方在社区健康服务中心的慢性病治疗中实施的可行性,为制定慢病长期处方的政策提出切实可行的建议。方法主要通过收集药师、全科医生和患者的调查问卷,对长期处方需求、了解程度、使用情况、开具情况、建议等方面进行分析。结果82.6%的被调查者了解长期处方,其中愿意使用长期处方的患者占75.5%,长期处方最主要的优点是节省时间、方便用药,缺点是调整用药不及时、与医生沟通减少,长期处方的患者服务需求主要是通过随访及时了解患者病情变化(94.0%)、提供用药咨询(87.0%)、药师处方审核(60.6%)。结论社区健康服务中心在获得患者、医师、药师一致认可的前提下实施慢病长期处方,可优化利用卫生资源,提高工作效率,但须加强长期处方的药物治疗管理,保证患者用药安全。

OBJECTIVE To explore the feasibility of long-term prescription of chronic diseases in the treatment of chronic diseases in community health service centers,and to put forward feasible suggestions for formulating long-term prescription of chronic diseases.METHODS By collecting questionnaires from pharmacists,general practitioners and patients,we analyzed the long-term prescription needs,understanding,usage,prescribing,and evaluation recommendations.RESULTS 82.6%of the respondents knew the long-term prescription.75.5%of them were willing to use the long-term prescription.The main advantages of the long-term prescription were time-saving and convenient use of drugs,while the disadvantages were delayed adjustment of medication and reduced communication with doctors.The service needs of patients with long-term prescription mainly included timely understanding of patients'condition changes through follow-up(94.0%),providing medication counseling(87.0%),and reviewing pharmacists'prescriptions(60.6%).CONCLUSION The long-term prescription of chronic disease in community health service centers can be optimized the use of health resources and be improved work efficiency on the premise of obtaining the unanimous recognition of patients,physicians and pharmacists.However,drug treatment management of long-term prescriptions must be strengthened to ensure the safety of patients'medication.
