血清丙氨酸氨基转移酶与糖尿病的关联性研究 被引量:3

摘要 目的探讨血清丙氨酸氨基转移酶(ALT)与糖尿病的关系,为早期预防糖尿病提供理论依据。方法采用2006—2015年北京电力医院体检中心的体检人群队列数据,根据基线血清ALT水平的四分位数将体检人群分为4组(Q_1~Q_4组),Q_1组作为参照。采用门诊随访方式,记录随访期间糖尿病发病率。采用二次推断函数方法分析血清ALT水平与糖尿病发生的关系。结果共入选研究对象11 601人,其中男性7 796人。随访期间共有553人发生糖尿病,发病率为4.8%。男性和女性的ALT水平不同[(30.32±22.77)U/L比(20.23±58.20)U/L,t=13.36,P<0.0001],因此分别进行分析。多变量二次推断函数分析显示,在控制了相关混杂因素后,男性ALT_Q_4的RR值为2.908(95%CI:2.236~3.783),女性ALT_Q_4的RR值为1.797(95%CI:1.121~2.882),因此血清ALT水平升高是糖尿病发生的危险因素。结论男性和女性的血清ALT水平均与糖尿病的发生呈正相关。 Objective To investigate the relationship between serum alanine aminotransferase(ALT)and diabetes,and to provide a theoretical basis for early prevention of diabetes.Methods Participants were enrolled from the Physical Examination Center of Beijing Electric Power Hospital from 2006 to 2015.Participants were divided into four groups(Q 1-Q 4 group)according to the quartile of the serum ALT level.Q 1 group was set as control group.The incidence of diabetes was recorded during clinic follow-up.The Quadratic Inference Functions(QIF)was used to explore the relationship between ALT levels and the occurrence of diabetes.Results A total of 11601 individuals were enrolled in the study,including 7796 men.The 553 people developed diabetes during the follow-up period,with an incidence of 4.8%.ALT levels were different in male and female groups[(30.32±22.77)U/L vs.(20.23±58.20)U/L,t=13.36,P<0.0001].According to the QIF analysis,RR for male was 2.908(95%CI:2.236~3.783),and RR for female was 1.797(95%CI:1.121~2.882).Increased ALT level was a risk factor for diabetes.Conclusions The level of serum ALT in men and women is positively correlated with the occurrence of diabetes mellitus.
