新型冠状病毒肺炎相关心肌损伤的诊疗进展

Advances in diagnosis and management of myocardial injury associated with coronavirus disease 2019

新型冠状病毒肺炎(COVID-19)患者除了典型的呼吸系统表现,也有一定比例的患者出现心脏受累的临床表现。常起病隐匿且恶化迅速,短期内可进展为急性左心衰竭、顽固性休克或致命性心律失常,因此早期识别,并根据患者所处疾病的不同阶段实施有针对性的治疗措施,对于降低COVID-19患者死亡率,改善重型及危重型患者的预后至关重要。本文将结合最新文献报道和作者团队的诊治经验对COVID-19相关心肌损伤的定义、发病机制、临床评估和治疗方案做一综述。

In addition to the typical respiratory manifestations,a certain proportion of patients with new coronavirus pneumonia(coronavirus disease 2019,COVID-19)also have clinical manifestations of cardiac injury.It is often hidden and deteriorates rapidly,and can progress to acute left heart failure,cardiac shock,or fatal arrhythmia in a short period of time.Therefore,early identification and targeted treatment measures carried out according to the different stages of the patient’s disease are essential to improve the prognosis of severe patients and decrease COVID-19 patient mortality.This article will review the definition,pathogenesis,clinical evaluation and management of COVID-19-related myocardial injury.
