摘要
目的探讨新型脐动静脉固定装置的设计及其在危重早产儿中的应用效果。方法回顾性分析2017年12月至2019年6月在新生儿监护室行脐动静脉置管的90例早产儿的临床资料,按入院时间分为对照组与试验组,每组45例。对照组采用常规荷包缝合胶布搭桥固定术,试验组采用新型脐动静脉固定装置固定法,比较两组置管操作时间、导管留置时间以及脱管、移位、出血、感染、血管痉挛、血栓、拔管困难、腹胀的发生情况。结果试验组置管操作时间短于对照组,试验组导管留置时间长于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论新型脐动静脉固定装置具有操作方便、固定效果好的优势,能够缩短置管操作时间,降低脱管、移位的发生率,安全性高。
Objective To investigate the design of a new umbilical artery and vein fixator and its clinical application in critically premature infants.Methods From December 2017 to June 2019,70preterm infants were divided into control group and observation group according to admission time,35 in each group.The control group was treated with umbilical artery and vein catheterization and fixed by conventional purse suture and tape.The observation group was treated with umbilical artery and vein catheterization and fixed by new umbilical artery and vein fixator.The operation time of catheterization,catheter retention time,the occurrence of detachment,displacement,bleeding,infection,vasospasm,thrombosis,difficulty in extubation and abdominal distension were compared between the two groups.Results The operation time of catheter placement in the experimental group was shorter than that in the control group,and the catheter retention time in the experimental group was longer than that in the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusions The application of the new umbilical artery and vein fixator is convenient and effective.It can shorten the operation time of catheterization,reduce the incidence of detachment and displacement,and does not increase the incidence of adverse complications.It is safe and worthy of clinical application.
