多元化护理干预模式在冠心病合并慢性阻塞性肺疾病患者围术期快速肺康复中的应用研究

Application of diversified nursing intervention in the perioperative rapid pulmonary rehabilitation of patients with coronary artery disease and chronic obstructive pulmonary disease(COPD)

目的研究多元化的肺康复干预措施在冠心病合并慢性阻塞性肺疾病(COPD)患者围术期的应用效果。方法选择2018年1月—2019年10月在本院行冠状动脉旁路移植术且合并COPD的符合纳入标准的患者共120例作为研究对象,随机分为实验组和对照组两组,每组各60例。对照组采用常规肺康复措施,实验组采用多元化的肺康复干预措施。对比两组患者在术后住院时间、气管插管时间、ICU入住时间、肺部并发症、患者满意度方面的效果。结果两组患者临床资料比较差异无统计学意义(P>0.05)。两组患者术后住院时间、呼吸机使用时间及ICU入住时间有明显差异,实验组均小于对照组,两组患者肺部并发症及满意度有明显差异(P<0.05)。结论多元化的肺康复干预措施可以减少冠状动脉旁路移植术合并COPD患者术后ICU入住时间和术后住院时间,减少肺部并发症的发生,能有效的促进患者的快速康复,值得在临床借鉴和推广。

Objective To study the application effect of diversified pulmonary rehabilitation interventions on patients with coronary artery disease complicated with chronic obstructive pulmonary disease.Methods From January 2018 to October 2019,120 patients with COPD and received coronary artery bypass grafting in our department and compliance with inclusion criteria were enrolled as study subjects,they were randomly divided into observation group and control group,and each group consisted of 60 cases.The control group received the routine pulmonary rehabilitation care,while the observation group were given diversified pulmonary rehabilitation interventions.The effects of the two groups on postoperative hospitalization time,tracheal intubation time,ICU stay time,pulmonary complications and patient satisfaction were compared.Results There was no significant difference between the two groups of patients in clinical data(P>0.05).The observation group were significantly shorter than the control group in post-hospitalization time,ventilator use time and ICU check-in time.There were significant differences in pulmonary complications and satisfaction between the two groups(P<0.05).Conclusions Diversified pulmonary rehabilitation interventions could reduce ICU stay and postoperative hospitalization time in patients underwent coronary artery bypass grafting combined with COPD,reduce the incidence of pulmonary complications,and effectively promote rapid recovery of patients,which is worthy of clinical reference and promotion.
