部分切断下胫腓后韧带浅层辅助复位治疗旋后外旋型踝关节骨折中合并Die-punch骨块的后踝骨折 被引量:8

摘要 目的:探讨部分切断下胫腓后韧带浅层辅助复位治疗旋后外旋(supination-external rotation,SER)型踝关节骨折中合并Die-punch骨块的后踝骨折的临床疗效与安全性。方法:2016年3月至2018年11月,采用部分切断下胫腓后韧带浅层辅助复位治疗SER型踝关节骨折中合并Die-punch骨块的后踝骨折患者20例,男14例、女6例,年龄23~67岁,中位数42岁。踝关节骨折根据Lauge-Hansen分型划分,SER型Ⅲ度3例、Ⅳ度17例。均为新鲜闭合性骨折,均累及胫骨腓切迹;合并内侧韧带断裂3例、内踝骨折14例,均合并外踝骨折;均未累及内踝后丘,均不合并严重多发伤、内科疾病及精神疾病。随访观察患者骨折愈合及并发症发生情况。末次随访时,采用美国足与踝关节协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝与后足评分标准评价综合疗效。结果:所有患者均获得随访,随访时间13~36个月,中位数16.5个月。切口均甲级愈合,术后14~16 d拆线。骨折均愈合,愈合时间10~16周,中位数12周。2例关节面移位约1 mm,其余均解剖复位。1例出现■长屈肌挛缩,经康复治疗后恢复正常;1例出现外踝螺钉激惹症状,术后13个月取出内固定物后症状消失;1例出现久行后踝关节周围肿胀并伴有轻度不适感,未行特殊处理。均无神经血管损伤、创伤性关节炎、骨折复位丢失及内固定失效等并发症发生。末次随访时AOFAS踝与后足功能评分(92.8±5.12)分,优17例、良3例。结论:采用部分切断下胫腓后韧带浅层辅助复位治疗SER型踝关节骨折中合并Die-punch骨块的后踝骨折,能够充分显露Die-punch骨块,骨折复位、愈合好,有利于踝关节功能恢复,且并发症少。
