药物联合无痛清宫终止不同孕周瘢痕子宫妊娠的疗效对比

Comparison of efficacy of medicine combined with painless currettage in terminating cesarean scar pregnancy at different gestational weeks

目的探讨药物联合无痛清宫终止不同孕周瘢痕子宫妊娠的疗效。方法收集2018年2月—2020年1月我院收治的90例瘢痕子宫妊娠患者的临床资料,根据不同孕周分为3组,将5周~6周设为A组(30例),7周~8周设为B组(30例),9周~10周设为C组(30例),再根据3组不同治疗方式,每组分别设置对照组及观察组,对照组予以无痛清宫治疗,观察组药物联合无痛清宫治疗,比较临床疗效。结果A组观察组总有效率较对照组高,但差异不显著(P>0.05);B、C组中观察组总有效率均较对照组显著升高(P<0.05);A组对照组总有效率高于B、C组对照组,差异有统计学意义(P<0.05);B组对照组总有效率高于C组对照组,但差异不显著(P>0.05)。结论瘢痕子宫妊娠在孕5周~6周采用单独药物或药物联合无痛清宫治疗效果均较优,且随着孕周增长,采用药物联合无痛清宫治疗效果更加显著。

Objective To investigate the efficacy of medicine combined with painless currettage in terminating cesarean scar pregnancy at different gestational weeks.Methods The clinical data of 90 patients with cesarean scar pregnancy who admitted in the hospital from February 2018 to January 2020 were retrospectively collected.They were divided into three groups according to different gestational weeks,5-6 weeks was set as A group(30 cases),7-8 weeks as B group(30 cases),9-10 weeks as C group(30 cases),and then according to different treatment methods of A,B,and C groups,control group and observation group were set in each group.Control group was given painless currettage,and observation group was treated with medicine and painless currettage.The clinical efficacy of six groups was compared.Results The total effective rate of observation group in A group was higher than that of control group,but the difference was not statistically significant(P>0.05);The total effective rate of observation group in B group and C group was higher than that of control group(P<0.05);The total effective rate of control group in A group was higher than that of control group in B group and C group(P<0.05);The total effective rate of control group in B group was higher than that of control group in C group,but the difference was not significant(P>0.05).Conclusion Medicine alone or in combination with painless currettage in cesarean scar pregnancy at 5-6 gestational weeks have better curative effects.However,with the increase of gestational weeks,the effect of medicine combined with painless currettage is more significant.
