聚焦解决模式对维持性血液透析患者容量管理及健康状况的影响

Effect of Solution-focused Model on Volume Management and Health Status of Maintenance Hemodialysis Patients

目的探讨聚焦解决模式对维持性血液透析患者容量管理及健康状况的影响。方法应用随机数字法将纳入本研究的80例维持性血液透析患者分为对照组和观察组,每组各40例。对照组采用传统说教模式进行干预,观察组采用聚焦解决模式进行干预。对比干预前和干预后3、6、12个月液体摄入依从性、容量控制达标率、高血压控制达标率,并采用健康调查简表(short-form36 health survey,SF-36)评估患者生活质量。结果干预后3个月观察组患者对液体摄入依从性高于对照组(t=-2.119,P=0.038),干预后6个月观察组患者容量管理达标率(χ~2=4.943,P=0.026)、高血压控制达标率(χ2=4.713,P=0.030)高于对照组。生活质量评分中观察组生理功能(t=-2.438,P=0.017)、生理职能(t=-3.555,P=0.001),一般健康状况(t=-2.973,P=0.004),精神健康(t=-2.483,P=0.015)分值高于对照组。结论采用聚焦解决模式可提高维持性血液透析患者容量控制管理能力,提高容量管理的效果,更好的对高血压进行控制,改善患者健康状况,从而提高其生活质量。

Objective To explore the effect of solution-focused model on volume management and health status of maintenance hemodialysis patients.Methods Using the random number method,80 maintenance hemodialysis patients included in this study were divided into control group and observation group,with 40 cases in each group.The control group used the traditional preaching mode to intervence,the observation group used the solution-focused model to intervene.Fluid intake compliance,volume control compliance,and hypertension control compliance were compared before and3,6,and 12 months after intervention,and patients′quality of life was assessed using the short-form36 health survey(SF-36).Results The compliance of the observation group was higher than that of the control group at 3 months after the intervention(t=-2.119,P=0.038),and the compliance rates of capacity management and hypertension control in the observation group at 6 months after intervention were higher than those of control group(χ2=4.943,4.713,P= 0.026,0.030).The scores of physiological function(t=-2.438,P=0.017),role physical(t=-3.555,P=0.001),general health status(t=-2.973,P=0.004)and mental health(t=-2.483,P=0.015)in the observation group were higher than those in the control group.Conclusion Adopting the solution-focused mode can improve the capacity management of maintenance hemodialysis patients,improve the effect of capacity management,better control hypertension,improve the health status of patients,and thus improve their quality of life.
