骨髓增殖性肿瘤患者不同预后与DNMT3A、MPL W515、JAK2 exon12基因突变的分析

摘要 目的[HT5"SS][HJ1.6mm]研究骨髓增殖性肿瘤患者不同预后与DNA甲基转移酶3A、MPL W515、JAK2 exon12基因突变的关系。方法选择骨髓增殖性肿瘤患者120例,根据核型分析结果显示,预后良好组患者56例,中度风险组患者42例,不良风险组患者22例,分析不同生存状态以及不同预后组患者基因突变情况之间的差异,分析患者的DNMT3A、MPL W515、JAK2 exon12基因突变与生存情况、预后风险之间的相关性。结果通过对不同预后风险患者的基因突变分析,随着患者的预后风险的升高,患者的DNMT3A、MPL W515、JAK2 exon12基因突变率显著升高;通过对患者的5年生存分析,生存患者90例,死亡患者30例,通过对不同生存期患者的基因突变情况分析,死亡组患者的DNMT3A、MPL W515、JAK2 exon12基因突变率显著高于生存组。患者的DNMT3A、MPL W515、JAK2 exon12基因突变率与生存情况、预后风险呈现正相关。结论骨髓增殖性肿瘤患者DNMT3A、MPL W515、JAK2 exon12基因突变可能与患者的预后相关,未来可做为患者诊断与治疗的重要靶点。 Objective To study the DNMT3A,MPL W515,JAK2 exon12 gene mutation types in patients with myeloproliferative tumor in different prognosis.Methods 120 patients with bone marrow proliferative tumors were enrolled in the study and the patients were treated.According to the karyotype analysis results,there were 56 patients in good prognosis group,42 patients in moderate risk group and 22 patients in poor risk group.The genetic processes of patients with different survival status and different prognosis were analyzed.The correlation between DNMT3A,MPL W515,JAK2 exon12 gene mutations,the survival and prognostic risk was analyzed.Results The mutation of DNMT3A,MPL W515 and JAK2 exon12 genes in patients with different prognostic risk increased significantly with the increasing of prognostic risk.After analysis of 5-year survival of patients,90 survivors and 30 deaths were found in the study.The mutation of DNMT3A,MPL W515 and JAK2 exon12 genes in patients with different prognostic risk were increased significantly.Analysis of the gene mutation showed that DNMT3A,MPL W515 and JAK2 exon12 genes mutations were significantly increased in the death group.The mutation of DNMT3A,MPL W515 and JAK2 exon12 genes were positively correlated with survival and prognosis risk.Conclusion DNMT3A,MPL W515,JAK2 exon12 genes mutation in patients with bone marrow proliferative tumors are significantly related to the prognosis.It can be used as important targets for diagnosis and treatment in the future.
