曲安奈德益康唑乳膏联合外耳道冲洗治疗真菌性外耳道炎的临床研究

Clinical study ontreating fungal external otitis by using pevisone combined with external auditory canal irrigation

目的探讨曲安奈德益康唑乳膏联合外耳道冲洗治疗真菌性外耳道炎的临床效果。方法选择2017年9月—2019年1月本院收治的真菌性外耳道炎患者74例作为研究对象,随机数表法分为对照组和观察组两组,每组各37例。对照组给予曲安奈德益康唑乳膏治疗,观察组在对照组基础上联合外耳道冲洗治疗,治疗1个月后比较两组患者疼痛情况、功能指数及日常生活能力、患者病情积分情况、患者治疗效果、不良反应情况。结果患者治疗前VAS、ODI、日常生活能力评分对比差异无统计学意义(P>0.05);治疗后观察组VAS、ODI评分,均低于对照组(P<0.05);观察组日常活动能力评分高于对照组(P<0.05);实验期间组内比较,治疗前患者病情积分无意义,患者治疗后的病情积分都低于疗前。组间行比较,观察组患者治疗后病情积分要低于对照组(P<0.05);治疗后观察组有效率为97.30%,对照组有效率为70.27%,观察组患者治疗效果显著高于对照组(P<0.05)。两组患者治疗过程中出现头晕、外耳道红肿、耳痒、疼痛等不良反应,观察组患者不良反应发生率要低于对照组(P<0.05)。结论将曲安奈德益康唑乳膏联合外耳道冲洗用于真菌性外耳道炎中可以减轻患者疼痛,改善患者流脓情况,减少不良反应发生,提高患者治疗效果,值得推广应用。

Objective To investigate the clinical effect of pevisone combined with external auditory canal irrigation in the treatment of fungal external otitis.Methods A total of 74 patients with fungal external otitis treated in our hospital from September 2017 to January 2019 were selected as the subjects.They were divided into the control group(n=37)and the observation group(n=37)according to random number table method.The control group was treated with pevisone,and the observation group was treated with external auditory canal irrigation on the basis of the control group.After 1 month of treatment,the two groups were compared in terms of pain,functional index and daily living ability,patients’disease scores,treatment effect and adverse reactions.Results Beforetreatment,the scores of VAS,ODI and daily living ability were not statistically significant(P>0.05).VAS and ODI scores of the observation group were lower than those of the control group after treatment(P<0.05).The daily activity ability score of the observation group was higher than that of the control group(P<0.05).During the experiment,comparison within groups indicated that the score of patients’condition before treatment was meaningless,and the score of patients’condition after treatment was lower than that before treatment.The scores of disease of patients in the observation group were lower than those in the control group after treatment(P<0.05).The effective rate was 97.30%in the observation group and it was 70.27%in the control group after treatment,and the therapeutic effect of the observation group was significantly higher than that of the control group(P<0.05).During the treatment,patients in the two groups presented adverse reactions such as dizziness,redness and swelling of external auditory canal,ear itching and pain.The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusions The application of pevisone combined with external auditory canal irrigation in the treatment of fungal external otitis can alleviate the pain,improve the condition of suppuration,reduce the occurrence of adverse reactions,and improve the therapeutic effect of patients,worth of promotion.
