肝脏脂肪沉积和胰腺脂肪沉积的关系及两者对空腹血糖水平的影响

Relationship between liver fatty deposition and pancreas fatty deposition and their effects on FBG level

目的探讨肝脏脂肪沉积和胰腺脂肪沉积的关系及两者对空腹血糖水平的影响。方法根据空腹血糖水平将95例成人住院患者分为高血糖组(n=24)与血糖正常组(n=71),均进行MRI Dixon序列扫描,测定肝脏脂肪分数(HFF)及胰腺脂肪分数(PFF)。分析HFF及PFF的相关性,以及影响空腹血糖水平升高的因素。结果高血糖组三酰甘油、HFF及PFF均高于血糖正常组(均P<0.05)。Logistic回归分析结果显示,仅HFF升高为空腹血糖升高的独立危险因子(P<0.05),经HFF校正后PFF与血糖水平升高的关系无统计学意义(P>0.05)。Pearson相关分析结果显示,在95例成人住院患者、血糖正常组、高血糖组中HFF与PFF均呈正相关(均P<0.05)。结论肝脏脂肪沉积和胰腺脂肪沉积密切相关,且肝脏脂肪沉积是空腹血糖水平升高的独立危险因素,而胰腺脂肪沉积与空腹血糖水平可能缺乏直接联系。

Objective To investigate the relationship between liver fatty deposition and pancreas fatty deposition and their effects on FBG level.Methods According to FBG level,95 hospitalized adult patients were divided into hyperglycemic group(n=24)and normal glucose group(n=71),and all patients were scanned with MRI Dixon sequence to measure their liver fat fraction(HFF)and pancreas fat fraction(PFF).The correlation between HFF and PFF,as well as the factors influencing FBG level elevation,was analyzed.Results The hyperglycemic group had higher TG,HFF and PFF than the normal glucose group(all P<0.05).Logistic regression analysis results showed that only HFF elevation was an independent risk factor for FBG elevation(P<0.05),no statistical significance was found between PFF and blood glucose level elevation after HFF correction(P>0.05).Pearson correlation analysis results revealed that HFF positively correlated with PFF in the 95 hospitalized adult patients,the normal glucose group and the hyperglycemic group(all P<0.05).Conclusion Liver fatty deposition and pancreas fatty deposition are closely related,and liver fat deposition is an independent risk factor for FBG elevation,but pancreas fatty deposition may not be directly related to FBG level.
