腹腔镜子宫肌瘤剔除术患者术后睡眠质量影响因素分析及护理

Analysis and Nursing of Influencing Factors of Postoperative Sleep Quality in Patients with Laparoscopic Hysteromyomectomy

目的:探讨腹腔镜子宫肌瘤剔除术患者术后睡眠质量影响因素及护理干预措施。方法:对2018年1月1日~2019年12月31日妇产科收治的184例子宫肌瘤剔除术患者采用匹兹堡睡眠质量指数量表(PSQI)进行术后睡眠质量调查,采用多因素回归分析法对腹腔镜子宫肌瘤剔除术患者术后睡眠质量相关影响因素进行分析并提出针对性护理措施。结果:184例子宫肌瘤剔除术患者中术后睡眠质量良好127例(占69.02%),睡眠质量不佳57例(占30.98%);单因素分析结果显示,年龄、疼痛程度、病程、被迫体位、病房环境、夜间治疗、胃纳情况、生活习惯、心理状况、自我效能感及社会支持均为影响腹腔镜子宫肌瘤剔除术患者术后睡眠质量的相关因素(P<0.05);多因素回归分析结果显示,疼痛程度、胃纳情况、心理状况及社会支持为影响腹腔镜子宫肌瘤剔除术患者术后睡眠质量的独立因素(P<0.05)。结论:针对患者疼痛程度、胃纳情况、心理状况及社会支持实施针对性护理措施,可改善患者的睡眠质量。

Objective:To explore the influencing factors of postoperative sleep quality and nursing intervention measures in patients with laparoscopic hysteromyomectomy.Methods:From January 1,2018 to December 31,2019,123 patients with uterine fibroids eliminate surgery admitted in the obstetrics and gynecology department were investigated with Pittsburgh sleep quality index scale(PSQI).Multi-factor regression analysis was used to analyze the influencing factors of postoperative sleep quality in patients undergoing laparoscopic hysteromyomectomy and targeted nursing measures were put forward.Results:Among the 123 patients undergoing hysteromyomectomy,85(69.11%)cases had good sleep quality after operation,while 38(30.89%)cases had poor sleep quality.Univariate analysis showed that age,pain degree,course of disease,forced posture,ward environment,night treatment,stomach condition,living habits,psychological status,self-efficacy and social support were all related factors affecting postoperative sleep quality of patients undergoing laparoscopic hysteromyomectomy(P<0.05).The results of multivariate regression analysis showed that pain degree,gastric condition,psychological status and social support were independent factors affecting postoperative sleep quality of patients undergoing laparoscopic hysteromyomectomy(P<0.05).Conclusion:According to the degree of pain,stomach,psychological status and social support of patients,the application of target nursing measures can improve the sleep quality of patients.
