摘要
目的:探讨腹腔镜胆总管探查术后胆总管一期缝合与T管引流的临床效果。方法:收集2016年1月~2019年2月本院收治的60例肝外胆管结石患者为观察组,并随机分组。T管引流组(n=30)采取腹腔镜胆总管探查术后T管引流,胆总管一期缝合组(n=30)实施腹腔镜胆总管探查术后胆总管一期缝合。分析手术操作的时间以及术后住院的总时间,治疗前后患者生活质量,术后使用镇痛药物的比例。结果:胆总管一期缝合组生活质量高于T管引流组,P<0.05。胆总管一期缝合组手术操作的时间以及术后住院的总时间短于T管引流组,P<0.05。胆总管一期缝合组术后使用镇痛药物的比例低于T管引流组,P<0.05。结论:肝外胆管结石患者实施腹腔镜胆总管探查术后胆总管一期缝合效果确切,可减少术后疼痛,缩短住院的时间和改善患者的生活质量。
Objective:To explore the clinical effect of primary suture of the common bile duct and T tube drainage after laparoscopic common bile duct exploration.Methods:From January 2016 to February 2019,60 patients with extrahepatic bile duct stones admitted to our hospital were collected as the observation group and randomly grouped.T-tube drainage group(n=30)received T-tube drainage after laparoscopic common bile duct exploration,and the common bile duct primary suture group(n=30)received primary suture after laparoscopic common bile duct exploration.Analyze the operation time and the total time of hospitalization after the operation,the quality of life of patients before and after treatment,and the proportion of analgesics used after operation.Results:The quality of life in the primary suture group of the common bile duct was higher than that in the T tube drainage group,P<0.05.The operation time and total hospitalization time in the primary suture group of the common bile duct were shorter than those in the T tube drainage group,P<0.05.The proportion of postoperative analgesics in the primary suture group of the common bile duct was lower than that in the T tube drainage group,P<0.05.Conclusion:In patients with extrahepatic bile duct stones,the primary suture of the common bile duct after laparoscopic common bile duct exploration is effective,which can reduce postoperative pain,shorten the hospital stay and improve the quality of life of the patients.
