北京电子病历互联互通中的患者信息保护研究

Research on patient information protection in Beijing electronic medical record interconnection

[目的]了解医患双方对于个人信息共享的认知与意愿,并结合对北京市的调研情况,提出针对性的建议,以实现电子病历互联互通和患者信息保护二者的共赢。[方法]采用自行设计的调查问卷进行现场调查,回收的数据用Epidata 3.1录入,并用SPSS 25.0软件进行统计分析。[结果]在信息共享时,患者认为信息泄露的主要原因是医院内部的泄露(148人次,占36.7%),认为提高医务工作者的素养(126人次,占31.1%)及加强司法监督(101人次,占24.9%)尤为重要。在授权方式的选择上,患者希望每次都要确认(167人次,占41.1%);医务工作者更希望签署一次性授权书后定期再确认(人次108,占38%)。[结论]电子病历互联互通在方便患者的同时,也使患者个人信息面临的风险加大。应加强医院的内部管理,完善授权程序;制定合理高效的共享方式并加强监督管理,以更好地保护患者个人信息。

Objective Understand the cognition and willingness of both doctors and patients on sharing personal information,and combine the actual situation in Beijing with investigations.In order to achieve a win-win situation for the interconnection of electronic medical records and the protection of personal information of patients,specific suggestions are made.Methods The self-designed questionnaire was used for on-site investigation,and the recovered data were entered with Epidata 3.1 and analyzed statistically by SPSS25.0 software.Results In information sharing,patients perceived that the cause of information disclosure was from hospital management(148 person-time,36.7%),improved the literacy of medical workers(126 person-time,31.1%)and strengthened judicial supervision(101 person-time,24.9%)that were especially important.In terms of the choice of authorization mode,patients wanted to confirm each time(167 person-time,41.1%),medical workers prefered to sign a one-time authorization letter and confirm it regularly(108 person-time,38%).Conclusions While convenient for patients,the interconnection of electronic medical records increases the risk of personal information.To better protect the patient's personal information,it is necessary to strengthen the internal management of the hospital,perfect the authorization procedure,formulate a reasonable and efficient way to share and strengthen the supervision and management.
