2015—2017年全国114家三级甲等医院青光眼治疗药物应用现状分析

Analysis of Application of Glaucoma Treatment Drugs in 114 GradeⅢLevel A hospitals in China from 2015 to 2017

目的:了解2015—2017年全国114家三级甲等医院青光眼治疗药物应用现状,旨在探讨我国青光眼药物治疗的发展趋势,同时为临床合理用药提供参考。方法:抽取114家医院门诊眼科和五官科诊断为青光眼或高眼压症患者的用药数据,对青光眼治疗药物应用情况、用药频度(defined daily dose system,DDDs)、限定日费用(defined daily cost,DDC)、年增长率以及青光眼禁忌慎用药物等进行分析评价。结果:中成药及β肾上腺素能受体拮抗剂使用量最大,临床信任度高;各类药物的DDDs及DDC相对稳定但有波动,其中复方制剂滴眼液的DDDs呈现逐年缓慢增加的趋势,而其DDC呈现逐年缓慢降低的趋势。真实世界中,青光眼患者存在使用青光眼禁忌慎用药物的情况。结论:临床青光眼治疗药物总体使用情况合理,但青光眼患者存在使用青光眼禁忌慎用药物的情况,还需进一步优化。

OBJECTIVE: To investigate the application status of glaucoma treatment drugs in 114 grade Ⅲ level A hospitals in China from 2015 to 2017,and to explore the development trend of glaucoma drug therapy in China,so as to provide reference for clinical rational drug use.METHODS: Medication data of patients diagnosed with glaucoma or ocular hypertension in outpatient ophthalmology and department of ENT in 114 hospitals were extracted to analyze and evaluate the application of glaucoma drugs,defined daily dose system( DDDs),defined daily cost( DDC),annual growth rate and drugs contraindicated for glaucoma.RESULTS: The consumption amount of traditional Chinese medicines and β-adrenergic receptor antagonists took the lead,with high levels of clinical confidence.The DDDs and DDC of various drugs were relatively stable but fluctuated,among which the DDDs of compound drug eye drops showed the trend of slow increase year by year,while the DDC showed the trend of slow decrease year by year.In the real world,there were cases of glaucoma patients using drugs that were contraindicated for glaucoma.CONCLUSIONS: The overall use of clinical glaucoma drugs is rationale, yet it is necessary to further optimize the use of drugs contraindicated for glaucoma in patients with glaucoma.
