2型糖尿病患者血清低清蛋白水平对糖化清蛋白检测结果的影响 被引量:6

Effect of Serum Low Albumin Level on the Detection of Glycosylated Albumin in Patients with Type 2 Ddiabetes Mellitus

摘要 目的探讨2型糖尿病患者血清低清蛋白水平对糖化清蛋白检测结果的影响。方法回顾性统计分析了82例2型糖尿病患者的实验室检查结果,按照实验室血清清蛋白<35 g/L为异常,其中血清清蛋白正常患者41例为对照组,血清低清蛋白患者41例为观察组;分别记录清蛋白(albumin,ALB)、糖化清蛋白(glycosylated albumin,GA)、空腹血糖(FBG)和糖化血红蛋白(HbA1c),并分别计算GA/ALB比值和GA/HbA1c比值,然后统计比较两组患者的ALB,GA,FBG,HbA1c,GA/ALB和GA/HbA1c,并分析GA与ALB,FBG,HbA1c,GA/ALB和GA/HbA1c的相关性。结果与对照组比较,FBG,HbA1c和GA/ALB差异均无统计学意义(t=0.087,0.074,0.924,均P>0.05),而ALB,GA和GA/HbA1c均显著降低,差异均有统计学意义(t=11.463,3.588,4.186,均P<0.05)。在对照组和观察组中GA与ALB的相关性均无统计学意义(r=-0.238,P=0.183;r=0.383,P=0.053);在对照组中,GA与FBG,HbA1c均有强相关性,差异均有统计学意义(r=0.758,P=0.000;r=0.884,P=0.000);但是在观察组中GA与FBG,HbA1c相关性差异均无统计学意义(r=0.157,P=0.444;r=0.309,P=0.125)。结论在2型糖尿病患者伴有血清低ALB时,检测的GA结果存在假性降低,故在使用GA监测2型糖尿病患者短期血糖控制情况时,需重视此影响。 Objective To investigate the effect of serum low albumin level on the detection of glycosylated albumin in patients with type 2 diabetes mellitus(T2DM).Methods The laboratory results of 82 T2DM patients were analyzed retrospectively,according to the laboratory serum albumin less than 35 g/L,41 patients with normal serum albumin were in the control group and 41 patients with low serum albumin were in the observation group.Albumin(ALB),glycosylated albumin(GA),fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c)were recorded,and ratio of the GA/ALB and GA/HbA1c were calculated respectively.Then the ALB,GA,FBG,HbA1c,GA/ALB and GA/HbA1c of the two groups were statistically compared,and the correlation between GA and ALB,FBG,HbA1c,GA/ALB,GA/HbA1c was analyzed.Results Compared with the control group,FBG,HbA1c and GA/ALB had no significant difference(t=0.087,0.074,0.924,all P>0.05),but ALB,GA and GA/HbA1c were significantly reduced(t=11.463,3.588,4.186,all P<0.05).There was no significant correlation between GA and ALB in the control group and observation group(r=-0.238,P=0.183;r=0.383,P=0.053).In addition,there was a strong correlation between GA and FBG,HbA1c in the control group(r=0.758,P=0.000;r=0.884,P=0.000),but there was no correlation between GA and FBG,HbA1c in the observation group(r=0.157,P=0.444;r=0.309,P=0.125).Conclusion There was a false decrease in the detection results of GA in T2DM patients with low serum albumin,so the inspectors needed to pay more attention to this effect when using GA to monitor the short-term blood glucose control in T2DM patients.
