单纯修补术与胃大部分切除术在急性胃穿孔治疗中的应用价值分析

Application value analysis of simple repair and subtotal gastrectomy in the treatment of acute gastric perforation

目的:比较胃大部分切除术与单纯修补术治疗急性胃穿孔的临床疗效。方法:2001年2月-2019年8月收治急性胃穿孔患者38例,随机法分为两组。对照组采用胃大部分切除术治疗;观察组采用单纯修补术治疗。比较两组临床疗效。结果:观察组手术时间、肛门排气时间、住院时间、术中出血量及并发症发生率均明显低于对照组,差异有统计学意义(P<0.05)。结论:胃大部分切除术与单纯修补术均可有效治疗急性胃穿孔,前者疗效肯定且无复发可能,但创伤大;后者安全性高、操作简易,但术后易复发,因此需结合实际合理选择。

Objective:To compare the clinical effect of subtotal gastrectomy and simple repair in the treatment of acute gastric perforation.Methods:From February 2001 to August 2019,38 patients with acute gastric perforation were selected and randomly divided into two groups.The control group was treated with subtotal gastrectomy.The observation group was treated with simple repair.We compared the clinical efficacy of the two groups.Results:In the observation group,the operation time,anal exhaust time,hospitalization time,intraoperative bleeding volume and complication rate were significantly lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion:Subtotal gastrectomy and simple repair can effectively treat acute gastric perforation,the effect of the former is positive,and there is no possibility of recurrence,but the trauma is large.The safety of the latter is high and the operation is simple,but it is easy to recur after operation,therefore,we need to make a reasonable choice based on the actual situation.
