应用超弹性Ni-Ti直丝扩弓矫治牙弓狭窄的临床应用

摘要 目的:观察应用超弹性Ni-Ti直丝扩弓矫治青少年生长发育高峰期牙弓狭窄的效果。方法:2015-2018年收治牙弓狭窄患者24例,分别应用方丝弓、MBT直丝弓、PASS矫正器治疗,扩弓因遵循细丝轻力的原则,先后以0.012、0.014英寸超弹性Ni-Ti直丝顺序治疗。结果:24例患者中,除1例因未能坚持戴保持器致前牙唇倾外,其余23例均达到预期目标。治疗后,上颌第一前磨牙间增值最大,为5.5 mm;下颌第一前磨牙间次之,为3.3 mm;下颌尖牙间最小,为1.7 mm;扩弓前后模型测量数据比较,差异有统计学意义(P<0.05)。结论:对生长发育高峰期青少年牙弓狭窄畸形患者应用超弹性Ni-Ti直丝遵循细丝轻力的矫治原则,顺应生长发育期的生理机制,缓慢扩弓,从而达到预期的矫治目标,疗效稳定。 Objective:To observe the effect of applying superelastic Ni-Ti straight wire to correct dental arch stenosis at the peak of adolescent growth and development.Methods:24 cases of dental arch stenosis were enrolled,they were treated with square wire arch,MBT straight wire arch and PASS corrector respectively,by following the principle of lightness of filaments,to treat in the order of 0.012 and 0.014 inch superelastic Ni-Ti straight wires.Results:Among the 24 patients,excepted for one case,the anterior lip was tilted due to failure to adhere to the retainer,the remaining 23 cases all met the expected target.After treatment,the spacing of the maxillary first premolars increased most,the numerical value was 5.5mm.The first mandibular premolars followed by was 3.3 mm.The smallest was mandibular incisors,the numerical value was 1.7mm.There was significant difference in the measurement data of the model before and after the expansion(P<0.05).Conclusion:The application of superelastic Ni-Ti straight wire in arch expansion of dental arch stenosis during the peak period of growth and development follows the principle of lightness of filaments,conforms to the physiological mechanism of growth and development,slowly expands the arch,so as to achieve the desired correction goal and the effect is stable.
