超声引导下细针穿刺抽吸活检技术辅助诊断5mm以上甲状腺结节的效果分析

Effect analysis of US-FNAB assisted diagnosis of thyroid nodules over 5 mm

目的:分析超声引导下细针穿刺抽吸活检(US-FNAB)技术辅助诊断5 mm以上甲状腺结节的效果。方法:2015-2018年在某医院收集5 mm以上甲状腺结节的US-FNAB检查结果,采集对应的细胞学结果和术后病理学结果(作为金标准)验证目标观察方法的吻合度。结果:收集到111个US-FNAB检查5 mm以上甲状腺结节的结果,标本满意度为89.2%;将满意标本96个按结节大小分为两组,A组为甲状腺结节5~10 mm的标本63个;B组为甲状腺结节>10 mm的标本33个。US-FNAB诊断,A组和B组准确度、灵敏度、特异度、假阳性率、假阴性率、阳性预测值、阴性预测值、Kappa值比较,差异无统计学意义(P>0.05)。结论:US-FNAB技术辅助诊断5 mm以上甲状腺结节的效果理想,特异度高,阳性预测值高,阴性预测值高,与金标准吻合度高。

Objective:To explore the effect of US-FNAB(ultrasound guided fine needle aspiration biopsy)assisted diagnosis of thyroid nodules over 5 mm.Methods:We collected US-FNAB results of thyroid nodules over 5 mm,we collected the corresponding cytological results and postoperative pathological results(as the gold standard)to verify the consistency of the target observation method.Results:111 US-FNAB inspection results of thyroid nodules over 5 mm were collected and the satisfaction rate of specimens was 89.2%.96 satisfactory specimens were divided into two groups according to the size of the nodules.Group A had 63 specimens with 5 to 10 mm thyroid nodules,and group B had 33 specimens with more than 10 mm thyroid nodules.There was no significant difference in accuracy,sensitivity,specificity,false positive rate,false negative rate,positive predictive value,negative predictive value and kappa value between group A and group B in US-FNAB diagnosis(P>0.05).Conclusion:The effect of US-FNAB assisted diagnosis of thyroid nodules over 5 mm is ideal,the specificity is high,the positive predictive value is high,the negative predictive value is high,and the coincidence with the gold standard is high.
