慢性心力衰竭患者动态心电图检查持续时间对心律失常检出率的影响

Effect of the duration of dynamic electrocardiogram on the detection rate of arrhythmia in patients with chronic heart failure

目的:研究慢性心力衰竭(CHF)患者动态心电图检查持续时间对心律失常检出率的影响。方法:随机选择本院2016年12月-2018年3月诊治的110例CHF患者为研究对象,连续72 h监测患者的动态心电图,比较监测24 h和72 h的室上性以及室性心律失常检出率,及24 h、72 h动态心电图对非持续性心动过速(NSVT)的检出率。结果:72 h动态心电图监测阵发性心房颤动、早搏、短阵房性心动过速(NSAT)、NSVT的72 h心电图检出率大于24 h(P<0.05);72 h动态心电图对纽约心脏协会(NYHA)心功能Ⅲ级、N-末端B型利钠肽原(NT-proBNP)水平>1000μg/L、射血分数保留的CHF患者的NSVT的检出率高于24 h(P<0.05)。结论:动态心电图持续时间适当延长可提高慢性心力衰竭患者的心律失常的检出率。

Objective:To study the effect of the duration of dynamic electrocardiogramon the detection rate of arrhythmia in patients with chronic heart failure(CHF).Methods:110 patients with CHF diagnosed and treated in the hospital between December 2016 and March 2018 were randomly selected,and their dynamic electrocardiogramwas monitored for 72 hours.The detection rates of supraventricular and ventricular arrhythmia after 24 hours and 72 hours of monitoring were compared.The detection rate of NSVT was compared between 24-hour and 72-hourdynamic electrocardiograms.Results:72 h Holter monitoring of paroxysmal atrial fibrillation,premature beats,nsat,NSVT,72 h ECG detection rate was greater than 24 h(P<0.05);2 h Holter monitoring of NYHA cardiac function classⅢ,NT proBNP level>1000μg/L,ejection fraction preserved CHF patients had higher detection rate of NSVT than 24 h(P<0.05).Conclusion:The detection rate of arrhythmia in patients with CHF can be improved by prolonging the duration of dynamic electrocardiogram appropriately.
