初次髋关节置换术中髋臼假体周围骨折的影响因素

Influencing factors of periacetabular fracture in primary hip arthroplasty

目的:探讨初次髋关节置换术中髋臼假体周围骨折的影响因素。方法:回顾性分析2015年2月至2019年4月该院收治的206例初次行髋关节置换术患者的临床资料,将其中22例发生髋臼假体周围骨折的患者设为观察组,将184例未发生髋臼假体周围骨折的患者设为对照组,分析两组患者性别、体质量指数、手术入路、骨质疏松等临床资料,总结导致初次髋关节置换术中髋臼假体周围骨折的影响因素。结果:单因素分析显示,两组性别、骨质疏松、假体类型和原发疾病相比,差异有统计学意义(P<0.05);Logistic回归性分析显示,女性、骨质疏松、生物型假体、存在髋关节发育不良均为导致初次髋关节置换术中出现髋臼假体周围骨折的影响因素(OR>1,P<0.05)。结论:女性、骨质疏松、生物型假体和髋关节发育不良均为初次髋关节置换术中出现髋臼假体周围骨折的影响因素。

Objective:To analysis influencing factors of periacetabular fracture in primary hip arthroplasty.Methods:A retrospective analysis of the clinical data of 206 primary hip arthroplasty cases of our hospital from February 2015 to April 2019 was performed.Among them,22 patients with periacetabular fractures were set as observation group,and 184 patients without periacetabular fractures were set as control group.The clinical data of the two groups of patients were analyzed,including gender,body mass index,surgical approach,and osteoporosis,and the factors that led to the periacetabular prosthesis fracture were summarized.Results:The univariate chi-square test showed that the differences in the gender,osteoporosis,prosthesis type and primary disease between the two groups were statistically significant(P<0.05).Logistic retrospective analysis showed that female,osteoporosis,and biological type implant and hip dysplasia were the influencing factors of periacetabular fractures in primary hip arthroplasty(OR>1,P<0.05).Conclusions:female,osteoporosis,and biological type implant and hip dysplasia are the influencing factors of periacetabular fractures in primary hip arthroplasty.
