TAPP与无张力疝修补术治疗腹股沟斜疝效果比较

Comparison of TAPP and tension-free hernioplasty in the treatment of oblique inguinal hernia

目的:探讨成人腹股沟斜疝中腹腔镜经腹膜前疝修补术(TAPP)与传统无张力疝修补术治疗的效果,找出最佳手术方式。方法:收集接受手术治疗的成年腹股沟斜疝患者98例,根据实施的手术方法分为传统无张力疝修补术组(对照组56例)和TAPP术组(观察组42例)。比较两组患者的手术时长、出血量、下床时间、住院天数,比较两组患者的住院期间尿潴留、血肿、切口感染等并发症的发生率。结果:观察组患者手术时长较对照组明显增加(P<0.05);观察组患者出血量、住院天数较对照组明显减少(P<0.05);观察组患者下床时间较对照组明显缩短(P<0.05);观察组患者并发症发生率低于对照组,差异具有统计学意义(P<0.05)。结论:与传统无张力疝修补术比较,在成人腹股沟斜疝患者手术治疗中,选择TAPP法能够显著缩短住院天数,提早下床时间,降低手术失血量,减少尿潴留、血肿、切口感染等并发症,有助于患者快速康复。

Objective:The efficacy of transabdominal preperitoneal herniorrhaphy(TAPP)with traditional tension-free hernioplasty of oblique inguinal hernia in adult was compared to determine the best surgical approach.Method:98 cases of adult oblique inguinal hernia who underwent surgery were collected.According to the operation methods,56 cases were divided into the traditional tension-free hernioplasty group(control group)and 42 cases into the TAPP group(observation group).compare the length of operation,amount of bleeding,time of getting out of bed and days of hospitalization and compare the incidence of complications such as urinary retention,hematoma and incision infection.Result:The operation time of the observation group was significantly longer than that of the control group(P<0.05).The amount of bleeding and hospitalization days in the observation group were significantly less than that in the control group(P<0.05).The time of getting out of bed in the observation group was shorter than that in the control group(P<0.05).The complication rate of the observation group were significantly less than the control group(P<0.05).Conclusion:TAPP can significantly reduce the length of hospital stay,the time of getting out of bed and the amount of blooding during surgery in adult patients with oblique inguinal hernia and reduce complications such as urinary retention,hematoma and incision infection and help patients recover quickly compared with traditional tension-free hernioplasty.
