结构式家庭疗法在结肠癌患者中的应用价值探讨

Structural family therapy in patients with colon cancer

目的探讨结构式家庭疗法(SFT)在结肠癌患者中的应用价值。方法纳入82例结肠癌患者为研究对象,采用随机数字表法将82例患者分为观察组(n=41,实施院外结构式家庭疗法干预)和对照组(n=41,实施院外常规随访)。分别在干预前后记录自我护理能力量表(ESCA)、癌症疲乏量表(CFS)及大肠癌生存质量量表(FACT-C)评分以评估者自我护理能力、癌性疲乏状态和生活质量。结果干预前,两组各项ESCA评分(健康知识水平、自我护理技能、自我概念及自我责任感)、CFS评分(躯体疲乏、情感疲乏及认知疲乏)和FACT-C评分(生理状况、社会家庭状态、情感状况、功能状况及附加关注)比较差异均无统计学意义(均P>0.05)。干预后,观察组各项ESCA评分及FACT-C评分均高于对照组,各项CFS评分均低于对照组,差异有统计学意义(均P<0.05)。结论 SFT用于结肠癌患者有助于改善患者癌性疲乏,提高自我护理能力和生活质量。

Objectives To investigate the effect of structural family therapy(SFT)in patients with colon cancer.Methods Eighty-two patients with colon cancer were recruited and randomly assigned to treatment group(n=41,received SFT)and control group(n=41,received routine follow-up).The followings were evaluated before and after intervention:patients’self-care ability measured by the exercise of self-care agency(ESCA)scale,fatigue measured by cancer fatigue scale(CFS),and quality of life measured by functional assessment of cancer therapy-colorectal(FACT-C).Results Before the intervention,the two groups did not differ in ESCA scale(health knowledge,self-care skills,self-concept,and self-responsibility),CFS score(physical,emotional,and cognitive fatigue),and FACT-C scores(physiological status,social and family status,emotional state,functional status and ad⁃ditional attention)(P>0.05).After the intervention,the treatment group had significantly higher ESCA scale and FACT-C score and lower CFS score than the control group(P<0.05).Conclusion SFT is effective for patients with colon cancer in improving patients’cancer fatigue,self-care skills,and quality of life.
