溶脂液在腔镜下乳腺癌改良根治术中的应用

Application of lipolytic solution in modified radical mastectomy of breast cancer under endoscopy

目的探讨溶脂液在腔镜下乳腺癌改良根治术中的应用效果。方法选取2018年6月~2019年9月我院收治的90例实施腔镜下乳腺癌改良根治术治疗的患者作为研究对象进行回顾性分析,根据患者是否使用溶脂液辅助治疗将其分为A组与B组,每组各45例。A组患者采用溶脂辅助下行乳腺癌腋窝淋巴结清扫治疗,B组患者常规保留肋间臂神经腋窝淋巴结清扫治疗,比较两组患者的手术相关指标、引流液情况以及术后并发症发生情况。结果A组患者的清扫时间、总手术时间、引流时间均短于B组,差异有统计学意义(P<0.05);A组患者的术中出血量少于B组,差异有统计学意义(P<0.05);A组患者的淋巴结清扫数目多于B组,差异有统计学意义(P<0.05)。两组患者术后2、3、4 d的引流量均少于本组术后1 d,且A组术后1、2、3、4 d引流量均少于B组,差异有统计学意义(P<0.05)。A组患者术后并发症总发生率低于B组,差异有统计学意义(P<0.05)。结论溶脂液在腔镜下乳腺癌改良根治术中的效果较好,能明显改善患者的手术相关指标,降低并发症发生率。

Objective To explore the application effect of lipolytic solution in modified radical mastectomy of breast cancer under endoscopy.Methods A total of 90 patients who underwent laparoscopic modified radical mastectomy for breast cancer treated in our hospital from June 2018 to September 2019 were selected as the research objects for retrospective analysis.The patients were divided into group A and group B according to whether they used lipolytic solution adjuvant therapy,45 cases in each group.Patients in group A were treated with lipolysis for axillary lymph node dissection for breast cancer,and patients in group B were treated with axillary lymph node dissection with preserving intercostal brachial nerves.The operation-related indicators,drainage fluid and postoperative complications were compared between the two groups.Results The cleaning time,total operation time,and drainage time in the group A were shorter than those in the group B,the differences were statistically significant(P<0.05);the intraoperative blood loss in the group A was less than that in the group B,and the difference was statistically significant(P<0.05);the number of lymph node dissections in group A was more than that in group B,the difference was statistically significant(P<0.05).The drainage fluid of the two groups of patients at 2,3,and 4 days after surgery was less than that in the group at 1 day after surgery,and the drainage fluid of group A was less than that in the group B at 1,2,3,and 4 days after surgery,the differences were statistically significant(P<0.05).The total incidence of postoperative complications in group A was lower than that in the group B,and the difference was statistically significant(P<0.05).Conclusion The lipolytic solution has a good effect in the modified radical mastectomy of breast cancer under laparoscopic surgery,which can significantly improve the operation-related indicators of patients and reduce the incidence of complications.
