CT三维重建联合B超在肾结石经皮肾镜取石患者中的应用价值分析

Application value analysis of CT three-dimensional reconstruction combined with B-ultrasound in patients with renal calculus percutaneous nephrolithotomy

目的探讨CT三维重建联合B超在肾结石经皮肾镜取石患者中的应用价值。方法选取2016年1月至2019年2月本院收治的肾结石患者80例作为研究对象,采用随机数表法分为两组,各40例。对照组行B超联合CT平扫引导下肾结石经皮肾镜取石术,观察组行CT三维重建联合B超引导下肾结石经皮肾镜取石术,比较两组患者手术相关指标、结石清除率及术后感染情况。结果观察组手术时间(58.74±4.42)min、术中出血量(187.46±21.08)m L和术中输液量(215.41±20.66)mL均低于对照组[(69.54±5.74)min(、241.49±24.57)mL、(416.17±36.41)mL],差异有统计学意义(P<0.05);观察组单次穿刺成功率(97.50%)和结石清除率(95.00%)均优于对照组(82.50%、80.00%),差异有统计学意义(P<0.05);观察组术后并发症发生率为7.50%,低于对照组的20.00%,但差异无统计学意义。结论实施CT三维重建联合B超引导下肾结石经皮肾镜取石术,可有效提高手术效果及结石清除率,且安全性更高。

Objective To explore the application value of CT three-dimensional reconstruction combined with B-ultrasound in patients with renal calculus percutaneous nephrolithotomy.Methods 80 renal calculi patients treated in our hospital from January 2016 to February 2019 were selected as the study subjects,and they were divided into two groups by random number table method,with 40 cases in each group.The control group underwent percutaneous nephrolithotomy under the guidance of B-ultrasound combined with CT plain scan,and the observation group underwent percutaneous nephrolithotomy under the guidance of CT three-dimensional reconstruction and B-ultrasound.The operation-related indexes,calculi removal rate and postoperative infection were compared between the two groups.Results The operation time(58.74±4.42)min,intraoperative blood loss(187.46±21.08)mL and intraoperative fluid infusion(215.41±20.66)mL of the observation group were all lower than those of the control group[(69.54±5.74)min,(241.49±24.57)mL,(416.17±36.41)mL],the difference was statistically significant(P<0.05).The single puncture success rate(97.50%)and calculi clearance rate(95.00%)of the observation group were better than those of the control group(82.50%,80.00%),the difference was statistically significant(P<0.05).The postoperative complication rate in the observation group was 7.50%,lower than 20.00%in the control group,but the difference was not statistically significant.Conclusion The implementation of CT three-dimensional reconstruction combined with Bultrasound guided percutaneous nephrolithotomy can effectively improve the surgical effect and calculi removal rate,and which is safer.
