全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折的疗效比较

Comparison of total hip arthroplasty and hemiarthroplasty for the treatment of femoral neck fractures in the elderly

目的比较全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折的疗效。方法选取本院2012年2月至2019年2月收治的76例老年股骨颈骨折患者作为研究对象,随机分为A组和B组,各38例。A组予以半髋关节置换术,B组予以全髋关节置换术。比较两组手术情况、术后髋关节恢复状况和治疗效果。结果 B组手术时间明显长于A组,术中出血量明显高于A组(P<0.05);B组术后髋关节恢复优良率为94.74%,明显高于A组的76.32%(P<0.05);B组治疗总有效率(97.37%)与A组(78.95%)比较差异有统计学意义(P<0.05)。结论在老年股骨颈骨折治疗中,全髋关节置换术虽然手术时间较长,术中出血量较多,但能有效改善患者髋关节功能,提高治疗效果,值得临床推广运用。

Objective To compare the efficacy of total hip arthroplasty and hemiarthroplasty in the treatment of femoral neck fractures in the elderly.Methods 76 cases of elderly patients with femoral neck fracture treated in our hospital from February 2012 to February 2019 were selected as the research objects and randomly divided into group A and group B,with 38 cases in each group.Group A underwent total hip arthroplasty and group B underwent total hip arthroplasty.The operation status,the recovery status of the hip joint after surgery,and the therapeutic effect were compared between two groups.Results The operation time of group B was significantly longer than that of group A.The intraoperative blood loss was significantly greater than that of group A(P<0.05).The excellent rate of hip joint recovery was 94.74%in group B,which was much higher than 76.32%in group A(P<0.05).The difference between the total effective rate of treatment in group B(97.37%)and group A(78.95%)was significant(P<0.05).Conclusion In the treatment of femoral neck fracture in the elderly,total hip arthroplasty has a longer operation time and more intraoperative blood loss,but it can effectively improve the hip joint function and improve the therapeutic effect.It is worthy of clinical application.
