地佐辛复合氟比洛芬酯术后自控静脉镇痛对老年全髋置换术患者血流动力学及不良反应的影响

Effect of patient-controlled intravenous analgesia with dizosin and flurbiprofen axetil on hemodynamics and adverse reactions in elderly patients undergoing total hip replacement

目的探讨地佐辛复合氟比洛芬酯术后自控静脉镇痛对老年全髋置换术患者血流动力学及不良反应的影响。方法选取2018年3月至2020年3月本院收治的接受全髋关节置换术治疗的患者54例为研究对象,按照随机数字表法分为对照组和观察组,各27例。两组患者采用盐酸罗哌卡因行腰硬联合麻醉。术后连接静脉镇痛泵,对照组采用地佐辛+昂丹司琼进行镇痛,观察组采用地佐辛+昂丹司琼+氟比洛芬酯进行自控镇痛。比较两组患者术后1、4、8、24 h的平均动脉压(MAP)、心率(HR)、视模拟评分法(VAS)以及不良反应。结果观察组术后1、4、8、24 h MAP、HR和VAS均低于对照组,差异具有统计学意义(P<0.05)。观察组不良反应发生率低于对照组,差异具有统计学意义(P<0.05)。结论地佐辛复合氟比洛芬酯术后自控静脉镇痛可使老年全髋置换术患者血流动力学保持稳定,且可减少不良反应的发生。

Objective To investigate the effect of patient-controlled intravenous analgesia(PCIA)with dizosin and flurbiprofen axetil on hemodynamics and adverse reactions in elderly patients undergoing total hip replacement.Methods 54 patients with total hip arthroplasty from March 2018 to March 2020 were selected as the study subjects.According to the random number table method,the patients were divided into control group and observation group,with 27 cases in each group.The two groups were given ropivacaine hydrochloride combined spinal-epidural anesthesia.After operation,intravenous analgesia pump was connected.The control group was treated with dizosin and ondansetron dissolved into normal saline for analgesia.The observation group was treated with dizosin,ondansetron and flurbiprofen ester integrated into normal saline for self-controlled analgesia.The mean arterial pressure(MAP),heart rate(HR),visual analogue scale(VAS)and adverse reactions at 1,4,8 and 24 hours after operation were recorded and compared between the two groups.Results The MAP,HR and VAS of the observation group at 1,4,8 and 24 hours after operation were lower than those of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion The patient-controlled intravenous analgesia with dizosin and flurbiprofen axetil can keep the hemodynamics stable and reduce the incidence of adverse reactions in elderly patients undergoing total hip replacement.
