宫颈癌患者外周血细胞中miRNA-216b的表达水平及临床意义

Expression and clinical significance of miRNA-216b in peripheral blood cells from patients with cervical cancer

目的探讨宫颈癌患者外周血细胞中miRNA-216b的表达水平及临床意义。方法采用实时荧光定量聚合酶链反应检测55例宫颈癌患者和30例健康者外周血细胞中miRNA-216b的表达水平,采用受试者工作特征(ROC)曲线和曲线下面积(AUC)分析miRNA-216b对宫颈癌的诊断效能。根据miRNA-216b相对表达量的不同,将宫颈癌患者分为miRNA-216b低表达组和miRNA-216b高表达组,比较两组患者的临床特征及生存预后。结果宫颈癌患者外周血细胞中miRNA-216b的相对表达量明显低于健康者,差异有统计学意义(P﹤0.01)。ROC曲线分析结果显示,miRNA-216b能够鉴别诊断宫颈癌患者和健康者,其AUC为0.865(95%CI:0.771~0.956,P﹤0.01)。miRNA-216b低表达组患者临床分期为Ⅲ~Ⅳ期、有淋巴结转移、术后复发的比例均高于miRNA-216b高表达组患者,差异均有统计学意义(P﹤0.05)。miRNA-216b高表达组患者的总生存情况优于miRNA-216b低表达组,差异有统计学意义(P﹤0.05)。结论宫颈癌患者外周血细胞中miRNA-216b表达下调,其可以作为辅助诊断宫颈癌和判断预后的重要标志物。

Objective To investigate the expression level of miRNA-216b in peripheral blood cells from patients with cervical cancer and its clinical significance.Method The expression level of miRNA-216b in peripheral blood cells from 55 patients with cervical cancer and 30 healthy subjects were detected by real-time fluorescence quantitative polymerase chain reaction.Receiver operating characteristic(ROC)curve and area under the curve(AUC)were used to analyze the diagnostic efficacy of miRNA-216b for cervical cancer.Patients with cervical cancer were divided into miRNA-216b low-expression group and miRNA-216b high-expression group according to the relative expression of miRNA-216b,and the clinical characteristics and survival of the two groups were compared.Result The relative expression of miRNA-216b in peripheral blood cells from patients with cervical cancer was significantly lower than that from healthy subjects,and the difference was statistically significant(P<0.01).Analysis of ROC curve results indicated that miRNA-216b could diagnose patients with cervical cancer from healthy subjects,with an AUC of 0.865(95%CI:0.771-0.956,P<0.01).The proportion of patients in the miRNA-216b low-expression group with clinical stage III-IV,lymph node metastasis and postoperative recurrence were higher than those in the miRNA-216b high-expression group,and the differences were statistically significant(P<0.05).The overall survival of patients in the miRNA-216b high-expression group was better than that of the miRNA-216b low-expression group,and the difference was statistically significant(P<0.05).Conclusion The expression of miRNA-216b in peripheral blood cells from patients with cervical cancer is down-regulated,and it could be used as an important marker to assist diagnosis and judge the prognosis of cervical cancer.
