阿立哌唑治疗氨磺必利诱导的高催乳素血症的疗效及最佳有效血药浓度范围研究

Efficacy of Aripiprazole in the Treatment of Amesulfoprim-Induced Hyperprolactinemia and Range of the Best Effective Blood Drug Concentration

目的:探讨阿立哌唑治疗氨磺必利诱导的高催乳素血症的疗效及最佳有效血药浓度范围。方法:选取佛山市顺德区伍仲珮纪念医院2019年收治的精神分裂症患者88例,采用随机数字表法分为氨磺必利组(给予氨磺必利治疗)和联合组(给予阿立哌唑+氨磺必利治疗)。观察两组患者治疗前后阳性和阴性症状量表(positive and negative syndrome scale,PANSS)评分、雌二醇(E2)水平,并结合血药浓度结果比较两组患者治疗前后体内催乳素(PRL)水平的差异。结果:治疗12周后,联合组、氨磺必利组患者PANSS评分分别为(69.88±7.393)、(69.77±8.815)分,差异无统计学意义(P>0.05);联合组、氨磺必利组患者E2水平分别为(325.94±107.721)、(346.03±125.826)pmol/L,差异无统计学意义(P>0.05);联合组患者血清PRL水平为(15.17±20.165)ng/ml,明显低于氨磺必利组的(110.40±31.224)ng/ml,差异有统计学意义(P<0.05)。治疗第12周时,联合组患者阿立哌唑的血药浓度为(236.97±99.227)ng/ml。结论:阿立哌唑可有效改善氨磺必利引起的PRL水平升高,且疗效明确,阿立哌唑的最佳有效血药浓度为150~350 ng/ml。

OBJECTIVE:To investigate the efficacy of aripiprazole in the treatment of amesulfoprim-induced hyperprolactinemia and range of the best effective blood drug concentration.METHODS:A total of 88 patients with schizophrenia admitted into Shunde Wu Zhong Pei Hospital in 2019 were extracted to be divided into the amisulpride group(treated with amisulpride)and combination group(treated with aripiprazole+amisulpride)via the random number table.Positive and negative syndrome scale(PANSS)score,estradiol(E2)level of two groups were observed before and after treatment,differences of the levels of prolactin(PRL)of two groups were compared before and after treatment before and after treatment.RESULTS:After treatment of 12 weeks,the PANSS scores of the combination group and the amisulpride group were respectively(69.88±7.393)and(69.77±8.815)scores,and the difference was not statistically significant(P>0.05).The levels of E2 in the combination group and the amisulpride group were respectively(325.94±107.721)and(346.03±125.826)pmol/L,and the difference was not statistically significant(P>0.05).The serum level of PRL in the combination group was(15.17±20.165)ng/ml,significantly lower than that of the amisulpride group(110.40±31.224)ng/ml,the difference was statistically significant(P<0.05).After treatment of 12 weeks,the blood concentration of aripiprazole in the combination group was(236.97±99.227)ng/ml.CONCLUSIONS:Aripiprazole can effectively reduce the increase of prolactin induced by amisulpride,and the curative effect is clear,the best effective blood concentration of aripiprazole is from 150 to 350 ng/ml.
