基于生物心理社会医学探析G-CBT配合丙戊酸镁缓释片对双相情感障碍的价值

An analysis of the value of G-CBT combined with magnesium valproate sustained-release tablets in bipolar disorder based on bio-psycho-social medical model

目的基于生物-心理-社会医学探究团体认知行为治疗(G-CBT)联合丙戊酸镁缓释片治疗双相情感障碍的价值。方法选取92例双相情感障碍患者,分成研究组和对照组各46例,对照组仅采用丙戊酸镁缓释片治疗,研究组在药物治疗基础上配合G-CBT治疗,从生物-心理-社会医学角度评估两组患者治疗效果。结果研究组的总有效率为93.48%,对照组为78.26%,研究组总有效率高于对照组(P<0.05)。治疗后两组BRMS、PANSS评分均较治疗前下降,研究组BRMS、PANSS评分均低于对照组(P<0.05)。治疗后两组SF-36评分均较各自治疗前升高(P<0.05),治疗后研究组SF-36除躯体疼痛外各维度评分均高于对照组(P<0.05)。治疗后研究组WMS记忆商数、瞬时记忆、短时记忆和长时记忆评分均高于对照组(P<0.05)。治疗后两组患者炎性因子水平均较治疗前下降(P<0.05),研究组IL-1β、TNF-α指标水平低于对照组(P<0.05)。结论G-CBT联合丙戊酸镁缓释片治疗双相情感障碍可更好地缓解症状、改善患者的生存质量,消除机体炎症,从生物、心理以及社会功能多方面进行改善,值得推广。

Objective To explore the value of group-cognitive behavioral therapy(G-CBT)combined with magnesium valproate sustained-release tablets in the treatment of bipolar affective disorder based on bio-psycho-social medical model.Methods A total of 92 patients with bipolar disorder were selected and divided into study group(46 cases)treated with magnesium valproate sustained-release tablets combined with G-CBT and control group(46 cases)treated only with magnesium valproate sustained-release tablets.The therapeutic effects of two groups were evaluated from the perspective of bio-psycho-social medical model.Results The effective rate in study group(93.48%)was higher than that in control group(78.26%)(P<0.05).After treatment,the total scores of BRMS and PANSS in both groups decreased significantly compared with those before treatment,and the total scores of BRMS,PANSS in study group were significantly lower than those in control group(P<0.05).After treatment,all factor scores of SF-36 in both groups increased significantly compared with those before treatment(P<0.05),and all factor scores except physical pain of SF-36 in study group were significantly higher than those in control group(P<0.05).After treatment,all factor scores of WMS in study group were significantly higher than those in control group(P<0.05).After treatment,all inflammatory factor levels in both groups decreased significantly compared with those before treatment(P<0.05),and levels of IL-1βand TNF-αin study group were significantly lower than those in control group(P<0.05).Conclusion G-CBT combined with magnesium valproate sustained-release tablets can better alleviate symptoms,improve life quality,eliminate inflammation of the body from various aspects of biological,psychological and social functions in patients with bipolar disorder,which is worthy of further promotion.
