感知婚姻冲突、安全感对青少年抑郁症的影响

Effects of perception of marital conflict and sense of security on depression in adolescents

目的探讨感知婚姻冲突、安全感与青少年抑郁症的关系。方法选取82例青少年抑郁症患者(研究组)和76名健康者(对照组),两组均施测儿童感知婚姻冲突量表(CPIC)、安全感问卷(SQ),研究组施测汉密尔顿抑郁量表(HAMD),对数据进行分析。结果研究组CPIC各因子分均高于对照组(P<0.05),SQ各因子分均低于对照组(P<0.05)。相关分析结果表明,CPIC各因子分与HAMD焦虑/躯体化、日夜变化、迟滞因子分均呈正相关(P<0.05),威胁认知与体重因子分呈负相关(P<0.05),威胁认知、自我归因与绝望感因子分均呈正相关(P<0.05);SQ各因子分与HAMD焦虑/躯体化、体重、迟滞、绝望感因子分均呈负相关(P<0.05),人际安全感与日夜变化因子分呈负相关(P<0.01)。冲突特征、威胁认知、自我归因对研究组抑郁程度有正向预测效应(P<0.05),可以解释HAMD方差变异的41.6%;人际安全感对研究组抑郁程度有负向预测效应(P<0.01),可以解释HAMD方差变异的27.8%。安全感在青少年感知婚姻冲突与抑郁程度之间起部分中介作用,中介效应占总效应的39.06%。结论感知婚姻冲突、安全感对青少年抑郁症有显著预测作用,安全感在青少年感知婚姻冲突与抑郁程度之间起部分中介作用。

Objective To explore the effects of perception of marital conflict and sense of security on depression in adolescents.Methods A total of 82 adolescents with depression in study group and 76 healthy controls in control group were selected and assessed with Children’s Perception of Interparental Conflict Scale(CPIC),Security Questionnaire(SQ)and Hamilton Depression Scale(HAMD).Results All factor scores of CPIC in study group were significantly higher than those in control group(P<0.05),and all factor scores of SQ in study group were significantly lower than those in control group(P<0.05).Correlation analysis showed that all factor scores of CPIC were positively correlated with factor scores of anxiety/somatization,diurnal variation,retardation of HAMD(P<0.05);threat cognition factor score was negatively correlated with weight factor score(P<0.05);factor scores of threat cognition and self attribution were positively correlated with hopelessness factor score(P<0.05).All factor scores of SQ were negatively correlated with factor scores of anxiety/somatization,weight,retardation,hopelessness of HAMD(P<0.05);interpersonal sense of security factor score was negatively correlated with diurnal variation factor score(P<0.01).Conflict feature,threat cognition,self attribution could positively predict the depression severity in study group(P<0.05),accounting for 41.6%of variance;and interpersonal sense of security could negatively predict the depression severity in study group(P<0.01),accounting for 27.8%of variance.Sense of security had partial mediated effect between CPIC and depression severity in adolescents,with a mediating effect of 39.06%of total effects.Conclusion Perception of marital conflict and sense of security are predictors of depression severity in adolescents.The sense of security is a mediated variable between marital conflict perception and depression severity in adolescents.
