分层分组教学模式在儿科住院医师规范化培训中的应用 被引量:11

摘要 目的探讨分层分组教学模式在儿科住院医师规范化培训中的实践情况,为提高儿科住院医师规范化培训质量提供新思路。方法自2018年8月起,遵循“分层递进,螺旋上升”的原则,我院儿科住院医师规范化培训基地将培训学员按年级分层,制定分层培训内容及目标,修订轮转计划,将带教教师根据亚专业合理分组,临床带教采用带教老师—三年级住院医师(组长)—二年级住院医师—一年级住院医师—实习医师的模式,明确各组员职责,改进教学方法,循序渐进,分层培训及考核。结果所有住培学员均参与分层分组,通过分层分组培训,学员学习任务更加明确,学习积极性增强;专业英语能力、专业技能及团队合作能力增强,同时在360度评价、中期考核、出科考核及年度考核中成绩都有显著提升,2016级学员结业考核通过率100%。分层分组教学模式满意度调查平均满意度达95.4%。结论分层分组教学模式在提高学员专科技能、强化医学人文素养、培养教学及科研能力上起到积极的作用,但在培训过程中遇到的如小组长的选拔及考核方式、学员个性化的培养等问题都是不容忽视的,因此分层分组教学模式值得进一步探讨。 Objective To explore the application of stratified and grouped teaching mode to the standardized training of resident pediatricians.Methods From August 2018 onwards,following the principle of"progressive stratification and spiral rise",in the standardized training base for resident pediatricians in our hospital,the trainees were stratified according to grades,stratified training contents and objectives were formulated,the rotation plans were revised,and the clinical teachers were grouped reasonably according to subspecialties.In clinical teaching,the stratified mode of clinical teachers-Grade Three resident pediatricians(team leader)-Grade Two resident pediatricians-Grade One resident pediatricians-intern pediatricians was adopted,with the responsibilities of each group member being clarified and the teaching methods being improved step by step.Thus,the training and assessment were carried out in a stratified way.Results All the resident trainees participated in the stratified and grouped mode.Through stratified and grouped training,the learning tasks of the trainees became clearer and their learning enthusiasm increased;their professional English ability,professional skills and team cooperation ability were enhanced;meanwhile,their scores in 360-degree evaluation,mid-term evaluation,departmental rotation examination and annual evaluation were all significantly improved.The passing rate of Grade 2016 trainees in the final achievement test was 100%.The average rate of satisfaction with the stratified and grouped teaching mode was 95.4%in the survey of the satisfaction degree.Conclusion Stratified and grouped teaching mode plays a positive role in improving the trainees'specialized skills,strengthening medical humanistic attainment,and cultivating teaching and scientific research ability.However,the problems encountered in the training process,such as the selection and assessment of team leaders and the personalized cultivation of students,cannot be ignored.Therefore,the stratified and grouped teaching mode deserves further exploration.
