摘要
目的:探讨二甲双胍和胰岛素泵联合使用治疗妊娠期糖尿病(GDM)的效果及对血清人软骨糖蛋白-39(YKL-40)与血清胱抑素C(cys C)水平的影响。方法:选取2017年3月-2019年5月本院收治的96例GDM患者作为研究对象,根据随机数字表法分为观察组和对照组,每组48例。观察组采用二甲双胍联合胰岛素泵治疗,对照组采用二甲双胍联合胰岛素皮下注射治疗。比较两组治疗效果、治疗前与治疗2个月后血糖相关指标、血清YKL-40和cys C水平。结果:观察组治疗总有效率为95.83%明显高于对照组的66.67%(P<0.05)。治疗2个月后,两组糖化血红蛋白(HbA1c)、空腹血糖(FPG)及餐后2 h血糖(2 h PG)水平均低于治疗前,且观察组均低于对照组(P<0.05)。治疗2个月后,两组YKL-40和cys C水平均低于治疗前,且观察组均低于对照组(P<0.05)。结论:二甲双胍联合胰岛素泵治疗GDM患者效果更佳,能够显著降低患者血清YKL-40和cys C水平,具有一定的应用价值。
Objective:To investigate the effect of Metformin combined with Insulin pump in the treatment of gestational diabetes mellitus(GDM)and its effect on serum human chondroglycoprotein-39(YKL-40)and serum cystatin C(cys C)levels.Method:A total of 96 patients with GDM admitted to our hospital from March 2017 to May 2019 were selected as study subjects.They were divided into observation group and control group according to the random number table method,with 48 patients in each group.The observation group was treated with Metformin combined with Insulin pump,while the control group was treated with Metformin combined with Insulin subcutaneously.Therapeutic effect effect,blood glucose related indexes,serum YKL-40 and cys C levels Before treatment and after 2 months of treatment were compared between the two groups.Result:The total effective rate in the observation group was 95.83%significantly higher than 66.67%in the control group(P<0.05).After 2 months of treatment,the levels of glycosylated hemoglobin(HbA1c),fasting blood glucose(FPG)and 2 h postprandial blood glucose(2 h PG)in both groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).After 2 months of treatment,the levels of YKL-40 and cys C in both groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).Conclusion:Metformin combined with Insulin pump has a better effect in the treatment of patients with GDM.It can significantly reduce the serum levels of YKL-40 and cys C in patients,which has certain application value.
