尿动力学检测联合膀胱尿道同步测压对女性OAB患者逼尿肌过度活动状态的评估价值探究 被引量:1

摘要 目的:探讨尿动力学检测联合膀胱尿道同步测压对女性膀胱过度活动症(OAB)患者逼尿肌过度活动(DO)状态的评估价值。方法:回顾性分析2017年6月-2019年6月本院收治的80例女性OAB患者的病例资料,并将其作为观察组,选取同期于本院健康体检中心进行体检的50例健康女性志愿者为健康组。两组均进行了尿动力学检测和膀胱尿道同步测压检查,比较两组的初始尿意容量(FDV)、残余尿量(PVR)、最大尿流率(MFR)。比较两组的最大尿道压(MUP)和最大尿道闭合压(MUCP),分析其在OAB中的表达特征。根据DO发生情况将80例女性OAB患者分为DO组和普通OAB组,比较两组尿动力学相关指标和膀胱尿道同步测压相关指标,通过Pearson直线相关性分析比较上述指标分别与DO发生情况的相关性。结果:观察组的FDV、MFR、MUP和MUCP水平均低于健康组,PVR水平高于健康组,差异均有统计学意义(P<0.05)。DO组的FDV、MFR、MUP和MUCP水平均低于普通OAB组,PVR水平高于普通OAB组,差异均有统计学意义(P<0.05)。Pearson直线相关性分析结果显示,DO发生率与MFR、MUP、MUCP水平均呈明显负相关性(P<0.05),与PVR水平呈明显正相关性(P<0.05)。结论:尿动力学检测联合膀胱尿道同步测压能够评估女性OAB患者的DO状态,具有一定的临床意义。 Objective:To investigate the value of urodynamics detection combined with synchronous bladder and urethra manometry in the evaluation of detrusor overactivity (DO) in women with overactive bladder (OAB).Method:The data of 80 female patients with OAB admitted to our hospital from June 2017 to June 2019 were retrospectively analyzed,and they were taken as the observation group,50 healthy female volunteers who underwent physical examination in the health examination center of our hospital during the same period were selected as the health group.Urodynamic detection and synchronous bladder and urethra manometry were performed in both groups,and first desire to void (FDV),post-void residual (PVR),and maximum urine flow rate (MFR) were compared between the two groups.Maximum urethral pressure (MUP) and maximum urethral closure pressure (MUP) were compared between the two groups,and their expression characteristics in OAB were analyzed.According to the occurrence of DO,80 female OAB patients were divided into the DO group and the general OAB group,and urodynamic indexes and synchronous bladder and urethra manometry indexes of the two groups were compared.The correlation between the above indicators and DO were compared through Pearson linear correlation analysis.Result:The levels of FDV,MFR,MUP and MUCP in the observation group were lower than those in the healthy group,the level of PVR was higher than that in the healthy group,with statistically significant differences (P<0.05).The levels of FDV,MFR,MUP and MUCP in the DO group were all lower than those in the OAB group,the level of PVR was higher than that of normal OAB group,the differences were statistically significant (P<0.05).Pearson linear correlation analysis showed that the incidence of DO was negatively correlated with MFR,MUP and MUCP levels (P<0.05),and positively correlated with PVR levels (P<0.05).Conclusion:Urodynamic examination combined with synchronous bladder and urethra manometry can evaluate the DO status of female OAB patients,which has certain clinical significance.
