三位一体延续护理干预对出院糖尿病患者血糖及健康教育依从性的影响

Effects of Trinity Continuous Nursing Intervention on Blood Glucose and Health Education Compliance of Discharged Diabetic Patients

目的:探讨三位一体延续护理干预对出院糖尿病患者血糖及健康教育依从性的影响。方法:选取2018年1月-2019年1月在本院就诊后出院的糖尿病患者共计120例,按随机数字表法将其均分为观察组和对照组,每组各60例。对照组采用传统家庭护理干预,观察组采用三位一体延续护理干预。对比两组患者血糖变化(包括FBG、2 h PBG及HbA1c水平),评估两组患者健康教育依从性(包括合理饮食,按医嘱服药,适当运动及定期复诊)。结果:两组患者接受1年护理后,观察组患者FBG、2 h PBG及HbA1c水平均显著低于对照组患者(P<0.05);观察组患者合理饮食,按医嘱服药,适当运动及定期复诊评分均显著低于对照组患者(P<0.05)。结论:对出院糖尿病患者实施三位一体延续护理干预能有效降低患者血糖水平,提高患者健康教育依从性,对患者的疾病起着积极的作用,具有广阔的运用价值,值得临床实践与运用。

Objective:To investigate the effect of trinity continuous nursing intervention on the blood glucose and health education compliance of discharged diabetic patients.Method:From January 2018 to January 2019,a total of 120 patients with diabetes were selected and discharged from our hospital.According to the random number table method,they were all divided into observation group and control group,with 60 patients in each group.The control group was used traditional home nursing intervention,observation group was used trinity continued nursing intervention.Blood glucose changes(including FBG,2 h PBG and HbA1c levels)in the two groups were compared,and the compliance of patients in the two groups with health education was evaluated(including reasonable diet,taking medicine according to doctor’s orders,proper exercise and regular follow-up).Result:After 1 year of nursing care,FBG,2 h PBG and HbA1c in the observation group were significantly lower than those in the control group(P<0.05).The scores of patients in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:The implementation of trinity continuous nursing intervention for discharged diabetic patients can effectively reduce the patient’s blood glucose level,improve the patient’s health education compliance,plays a positive role in the patient’s disease,has broad application value,worthy of clinical practice and application.
