科学配餐指导对GDM孕产妇中晚期超高体重及妊娠结局的指导意义

The Significance of Scientific Dietary Guidance of High Body Weight and Pregnancy Outcome in Middle and Late Maternal Pregnant Women with GDM

目的:观察并分析妊娠期糖尿病(gestational diabetes mellitus,GDM)孕产妇接受科学配餐指导对其孕中晚期超高体重及妊娠结局的影响。方法:选取2018年1月-2019年12月期间在本院分娩的60例GDM孕产妇,使用数字随机表法将其分为对照组与研究组,每组各30例。对照组接受常规干预,研究组接受常规干预联合科学配餐指导,比较两组分娩前后不同时间段血糖情况、孕期不同阶段体重增长情况及妊娠结局。结果:分娩前,两组血糖各指标数值比较,差异均无统计学意义(P>0.05),分娩时研究组血糖各指标数值均低于对照组(P<0.05)。研究组孕期不同阶段体重增加量明显低于对照组(P<0.05)。研究组正常妊娠率高于对照组(P<0.05),而剖宫产率、妊娠期高血压疾病发生率、产后出血率、巨大儿率均低于对照组(P<0.05)。结论:GDM孕产妇在常规干预基础上接受科学配餐指导,能够有效控制其孕产期体重与血糖水平,降低不良妊娠结局者、发生率,提高正常妊娠率,该方式在临床上应加大推广使用力度。

Objective:To observe and analyze the impact of gestational diabetes mellitus(GDM)on gestational hyperweight and outcome in middle and late maternal.Method:A total of 60 GDM pregnant women who parturition in our hospital from January 2018 to December 2019 were divided into control and study groups by using the method of digital random table,there were 30 patients in each group.They received routine intervention,and routine intervention was combined with scientific dietary guidance.Result:There were no significant difference between the two groups in the indexes of blood glucose before delivery(P>0.05),and the indexes of blood glucose in the study group were lower than those in the control group(P<0.05).The weight gain at different stages of pregnancy in the study group was significantly lower than that in the control group(P<0.05).The rate of normal pregnancy in the study group was higher than that in the control group(P<0.05),while the rate of production planing palace,pregnancy hypertension,postpartum hemorrhage and macromacrosia were all lower than those in the control group(P<0.05).Conclusion:On the basis of routine intervention,GDM pregnant women receive scientific dietary guidance,which can effectively control their weight and blood glucose level during pregnancy,reduce adverse pregnancy outcome and incidence,and improve normal pregnancy rate.This method should be promoted and applied in clinical practice.
