摘要
目的检测慢性牙周炎患者龈沟液中乳铁蛋白(LF)的浓度,并分析其与炎性反应及T淋巴细胞亚群的关系。方法选择2017年2月至2018年8月收治的慢性牙周炎患者73例为研究组,另外选择牙周健康志愿者57例为对照组。对比治疗前慢性牙周炎患者、健康者的龈沟液中LF浓度、炎性因子及T淋巴细胞亚群水平,同时比较慢性牙周炎患者治疗前后的牙周检查指标、LF浓度、炎性因子及T淋巴细胞亚群指标变化,采用Pearson相关分析方法分析慢性牙周炎患者龈沟液中LF浓度与炎性因子、T淋巴细胞亚群指标的相关性。结果研究组患者治疗后探诊深度(PD)、附着丧失(AL)、牙龈出血指数(BI)、菌斑指数(PLI)以及探诊出血指数(BOP)均较治疗前降低(P<0.05)。与对照组比较,研究组治疗前龈沟液LF浓度、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C-反应蛋白(hs-CRP)、CD3^+、CD4^+及CD4^+/CD8^+水平均明显升高,而CD8^+水平显著下降,差异均有统计学意义(P<0.05);研究组治疗后龈沟液LF浓度、IL-6、TNF-α、hs-CRP、CD3^+、CD4^+及CD4^+/CD8^+水平均明显低于治疗前,而CD8^+水平显著高于治疗前,差异均有统计学意义(P<0.05);治疗前慢性牙周炎患者龈沟液中LF浓度与IL-6、TNF-α、hs-CRP、CD3^+、CD4^+及CD4^+/CD8^+呈正相关(P<0.05),而与CD8^+呈负相关(P<0.05)。结论LF与慢性牙周炎患者的炎性因子、T淋巴细胞亚群关系密切,可能参与了慢性牙周炎的发生和发展过程,检测LF有助于评估患者的疗效。
Objective To investigate the correlation between lactoferrin(LF)concentration and inflammatory reaction as well as T lymphocyte subsets in gingival crevicular fluid of patients with chronic periodontitis.Methods A total of 73 patients with chronic periodontitis who were admitted and treated in our hospital from February 2017 to August 2018 were enrolled as research group,and the other 57 healthy volunteers were enrolled as control group.The levels of LF,inflammatory factors and T lymphocyte subsets in gingival crevicular fluid before and after treatment were observed and compared between the two groups.Pearson correlation analysis was used to analyze the correlation between LF concentration and inflammatory reaction as well as T lymphocyte subsets in gingival crevicular fluid of patients with chronic periodontitis.Results After treatment,the probing depth(PD),attachment loss(AL),bleeding index(BI),plaque index(PLI)and bleeding on probing(BOP)in research group were significantly decreased(P<0.05).As compared with those in control group,the levels of LF,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),hypersensitive C reactive protein(hs-CRP),CD3^+,CD4^+ and CD4^+/CD8^+ in gingival crevicular fluid of patients in research group before treatment in research group were significantly increased,however,the levels of CD8^+ were decreased significantly(P<0.05).After treatment the levels of LF,IL-6,TNF-α,hs-CRP,CD3^+,CD4^+ and CD4^+/CD8^+ in gingival crevicular fluid of patients in research group were significantly lower than those before treatment,while the CD8^+ levels were significantly higher than those before treatment(P<0.05).Moreover the levels of LF in gingival crevicular fluid of patients before treatment were positively correlated with those of IL-6,TNF-α,hs-CRP,CD3^+,CD4^+ and CD4^+/CD8^+(P<0.05),but,which were negatively correlated with those of CD8^+(P<0.05).Conclusion LF is closely related to inflammatory factors and T lymphocyte subsets in patients with chronic periodontitis,which may be involved in the occurrence and development chronic periodontitis,therefore,the detection of LF is helpful to evaluate the therapeutic effects.
