新生儿细菌感染性肺炎外周血自然杀伤细胞、淋巴细胞亚群与病程进展的相关性研究 被引量:12

Correlation between the natural killer cells,lymphocyte subsets in the peripheral blood and the progression of the disease in neonates with bacterial infectious pneumonia

摘要 目的探讨新生儿细菌感染性肺炎外周血自然杀伤细胞和淋巴细胞亚群与病程进展的关系。方法收治的92例新生儿细菌感染性肺炎患者根据外周血白细胞的数量分为轻度感染组(n=58)和重度感染组(n=34),选取同期30例健康新生儿为正常对照组。采用流式细胞仪测定3组外周血自然杀伤细胞、淋巴细胞亚群以及白介素-4(IL-4)、γ-干扰素(IFN-γ)水平,采用酶联免疫吸附实验(ELISA)测定3组受试者降钙素原、C-反应蛋白以及IgA、IgG、IgM等免疫球蛋白含量。结果正常对照组受血清降钙素原、C-反应蛋白、IL-4表达水平明显低于轻度感染组和重度感染组(P<0.05),轻度感染组明显低于重度感染组(P<0.05)。正常对照组外周血自然杀伤细胞及其亚群表达水平明显高于低于轻度感染组和重度感染组,轻度感染组明显高于重度感染组(P<0.05)。正常对照组CD4^+表达水平明显高于轻度感染组和重度感染组(P<0.05),CD19+表达水平明显低于轻度感染组和重度感染组(P<0.05);轻度感染组CD4^+表达水平明显高于重度感染组、CD19+表达水平明显低于重度感染组(P<0.05)。正常对照组IgM含量明显低于轻度感染组和重度感染组,轻度感染组明显低于重度感染组(P<0.05)。结论新生儿细菌感染性肺炎患者外周血自然杀伤细胞及淋巴细胞亚群表达水平异常,且异常程度与病程进展相关。 Objective To investigate the correlation between the natural killer cells,lymphocyte subsets in the peripheral blood and the progression of the disease in neonates with bacterial infectious pneumonia.Methods A total of 92 neonates with bacterial infectious pneumonia were divided into mild infection group(n=58)and severe infection group(n=34)according to the number of peripheral blood leukocytes,at the came time,the other 30 healthy newnates delivered in our hospital were enrolled as the normal control group.Flow cytometry was used to determine the levels of peripheral blood natural killer cells and lymphocyte subsets,and thesrum levels of interleukin-4(IL-4)and interferon-γ(IFN-γ)in the three groups,and enzyme-linked immunosorbent assay(ELISA)was used to detect the levels of procalcitonin,C-reactive protein(CRP),and immunoglobulins including IgA,IgG,and IgM in the three groups.Results The expression levels of procalcitonin,CRP and IL-4 in normal control group were significantly lower than those in mild infection group and severe infection group(P<0.05),and the levels in mild infection group were significantly lower than those in severe infection group(P<0.05).The expression levels of peripheral blood natural killer cells and their subgroups in normal control group were significantly higher than those in mild infection group and severe infection group,moreover,which in mild infection group were significantly higher than those in severe infection group(P<0.05).The CD4^+expression levels in normal control group were significantly higher than those in mild infection group and severe infection group(P<0.05),however,the expression levels of CD19+were significantly lower than those in mild infection group and severe infection group(P<0.05).The expression levels of CD4^+in mild infection group were significantly higher than those in severe infection group,while the expression levels of CD19+were significantly lower than those in severe infection group(P<0.05).The IgM levels in normal control group were significantly lower than those in mild infection group and the severe infection group,moreover,which in mild infection group were significantly lower than those in severe infection group(P<0.05).Conclusion The expression levels of peripheral blood natural killer cells and lymphocyte subsets in neonates with bacterial infectious pneumonia are abnormal,and the abnormality degree is related to the progression of the disease.
