甲状旁腺激素水平与慢性肾功能衰竭患者残余肾功能相关参数的关系研究

Relationship between Parathyroid Hormone Level and Residual Renal Function Related Parameters in Patients with Chronic Renal Failure

目的:探讨甲状旁腺激素(PTH)水平与慢性肾功能衰竭(CRF)患者残余肾功能相关参数的关系。方法:选取2019年9月-2020年3月在本院人工肾及肾内科接受治疗的CRF患者60例。根据患者血液中的PTH水平分为A组33例和B两组27例,A组患者PTH>53 pmol/L,B组患者PTH≤53 pmol/L。比较两组患者的PTH水平、残余肾功能相关参数,并进行Spearman相关性分析。结果:两组Hb、Alb、P水平比较,差异均无统计学意义(P>0.05)。B组PTH、BUN、Scr、Ca、RBP均低于A组,Ccr高于A组,差异均有统计学意义(P<0.05)。Spearman相关性分析结果显示,PTH水平与患者的残余肾功能相关参数BUN、Scr、RBP均呈正相关(P<0.05),与Ccr、Ca均呈负相关(P<0.05)。PTH与BUN、Scr、Ccr、RBP、Ca存在直线相关性(R^2=0.242、0.216、0.207、0.245、0.256)。结论:PTH水平在一定程度能够反应CRF患者的残余肾功能,有助于评估疾病进展情况,治疗过程中应充分重视,并采取有效的干预方案。

Objective:To explore the relationship between parathyroid hormone(PTH)level and residual renal function(RRF)related parameters in patients with chronic renal failure(CRF).Method:Sixty patients with CRF who received treatment in the department of artificial kidney and nephrology in our hospital from September 2019 to March 2020 were selected.According to the PTH level in the blood,the patients were divided into group A of 33 cases and group B of 27 cases,PTH>53 pmol/L in group A,PTH≤53 pmol/L in group B.The PTH level and residual renal function related parameters of two groups were compared,Spearman correlation analysis was performed.Result:There were no significant differences in Hb,Alb and P levels between two groups(P>0.05).PTH,BUN,Scr,Ca and RBP of group B were all lower than those of group A,and Ccr was higher than that of group A,there were statistically significant differences(P<0.05).Spearman correlation analysis showed that PTH level was positively correlated with residual renal function parameters BUN,Scr and RBP(P<0.05),while negatively correlated with Ccr and Ca(P<0.05).PTH had linear correlation with BUN,Scr,Ccr,RBP and Ca(R^2=0.242,0.216,0.207,0.245,0.256).Conclusion:PTH level can reflect the residual renal function of CRF patients to a certain extent,which is helpful to evaluate the disease progression of patients.Sufficient attention should be paid to the treatment,and effective intervention schemes should be adopted.
