宫腔镜电切术联合曼月乐环治疗复发性子宫内膜息肉的效果

Effect of Hysteroscopy Electroresection Combined with Mirena Ring in the Treatment of Recurrent Endometrial Polyps

目的:探讨宫腔镜电切术联合曼月乐环治疗复发性子宫内膜息肉的效果。方法:选取2013年1月-2014年12月于本院就诊的复发性子宫内膜息肉患者80例,按照随机数字表法将其分为对照组与研究组,每组40例。对照组行宫腔镜电切术联合地屈孕酮治疗,研究组行宫腔镜电切术联合曼月乐环治疗。比较两组治疗前后月经情况、子宫内膜厚度及性生活质量,比较两组随访5年期间子宫内膜息肉复发率及并发症发生率。结果:术后1、3个月,两组月经量、月经周期及视觉模拟评分法(visual analogue score,VAS)评分均低于治疗前,且研究组均低于对照组(P<0.05)。术后1年,研究组子宫内膜厚度低于对照组(P<0.05)。术后3个月,两组各项女性性功能评估量表(brief index of sexual function for women,BISF-W)评分均高于治疗前,且研究组均高于对照组(P<0.05)。研究组随访5年期间子宫内膜息肉复发率与并发症发生率均低于对照组(P<0.05)。结论:宫腔镜电切术联合曼月乐环治疗复发性子宫内膜息肉能显著改善患者月经周期,提高性生活质量,抑制子宫内膜息肉复发,值得临床推广。

Objective:To investigate the effect of hysteroscopy electroresection combined with Mirena ring in the treatment of recurrent endometrial polyps.Method:A total of 80 patients with recurrent endometrial polyps admitted to our hospital from January 2013 to December 2014 were selected.They were divided into control group and study group according to the random number table method,40 patients in each group.The control group was treated with hysteroscopy electroresection combined with Dydrogesterone,while the study group was treated with hysteroscopy electroresection combined with Mirena ring.The menstrual conditions,endometrial thickness and sexual quality of the two groups were compared before and after treatment.The recurrence rate of endometrial polyp and complication rate during 5 years of follow-up were compared between the two groups.Result:At 1 and 3 months after surgery,the menstrual volume,menstrual cycle and visual analogue score(VAS)score in two groups were lower than those of before treatment,and the study group were lower than those in the control group(P<0.05).1 year after surgery,the endometrial thickness of the study group was lower than that of the control group(P<0.05).3 months after surgery,all brief index of sexual function for women(BISF-W)scores in two groups were higher than those of before treatment,and the study group were higher than those in the control group(P<0.05).The recurrence rate of endometrial polyp and complication rate during 5 years of follow-up in the study group were lower than those in the control group(P<0.05).Conclusion:Hysteroscopy electroresection combined with Mirena ring in the treatment of recurrent endometrial polyps can significantly improve the menstrual cycle,improve the quality of sexual life,inhibit the recurrence of endometrial polyps,which is worthy of clinical promotion.
