毫针刺络温针灸联合循经推拿法治疗上肢淋巴水肿临床研究

Clinical Study of Needling Collaterals and Needle-Warming Moxibustion Combined with Tuina along Meridian in the Treatment of Upper Limb Lymphedema

目的:探讨毫针刺络温针灸联合循经推拿法治疗上肢淋巴水肿的临床疗效。方法:选取本院周围血管科在2018年5月—2019年5月收治的40例上肢淋巴水肿患者,随机分为研究组和对照组,各20例。研究组患者给予毫针刺络温针灸联合循经推拿法治疗,对照组给予氢氯噻嗪片口服治疗,观察两组患者临床疗效、治疗前后患-健侧臂围差、主观症状评分、睡眠质量评分及淋巴造影峰值时间和相对信号强度,两组均以10 d为1个疗程,共治疗3个疗程。结果:研究组临床总有效率优于对照组(P<0.05);两组治疗后患-健侧臂围差均有所改善,但研究组改善优于对照组(P<0.05);治疗后主观症状评分研究组低于对照组(P<0.05);治疗后睡眠质量评分研究组优于对照组(P<0.05);两组治疗前后淋巴造影峰值时间及相对信号强度均改善,且研究组改善优于对照组(P<0.05)。结论:毫针刺络温针灸联合循经推拿法治疗上肢淋巴水肿临床疗效确切,可以有效缓解上肢肿胀、改善肢体功能、改善患者生活质量,值得临床借鉴。

Objective:To explore the clinical efficacy of needling collaterals and needle-warming moxibustion combined with Tuina along meridians in the treatment of upper limb lymphedema.Methods:40 patients with upper limb lymphedema were randomly divided into the study group(n=20)and the control group(n=20).The patients in the study group were treated with needling collaterals and needle-warming moxibustion combined with Tuina along meridians,whereas the patients in the control group were treated with oral hydrochlorothiazide.Ten days were taken as one course of treatment,the treatment duration was three courses.The total effective rate,the difference of arm circumference,subjective symptom score,sleep quality score,peak time and relative signal intensity of lymphography were observed before and after the treatment in the two groups.Results:The total curative effect of the study group was significantly better than that of the control group(P<0.05).The difference of arm circumference was improved after the treatment,and the improvement in the study group was more significant(P<0.05).The subjective symptom score in the study group was lower than that in the control group(P<0.05),and the sleep quality score in the study group was significantly higher than that in the control group after the treatment(P<0.05).The peak time and relative signal intensity of lymphography were improved in the two groups after the treatment,and the improvements in the study group were more significant(P<0.05).Conclusion:The therapy of needling collaterals and needle-warming moxibustion combined with Tuina along meridians is effective in treating upper limb lymphedema,which can effectively relieve upper limb swelling,improve limb function and improve life quality of the patients.
