复杂性肾结石应用单通道与双通道经皮肾镜进行治疗的效果与并发症探讨

The Effect and Complications of Single-channel and Double-channel Percutaneous Nephroscopy in the Treatment of Complicated Kidney Stones

目的:探讨复杂性肾结石应用单通道与双通道经皮肾镜碎石术(PCNL)进行治疗的效果与并发症发生情况。方法:选择2016年1月-2020年1月在笔者所在医院治疗的60例复杂性肾结石患者开展本次研究,随机分为参照组和试验组,每组30例,参照组给予单通道PCNL进行治疗,试验组给予双通道PCNL进行治疗,对比两组治疗效果。结果:两组都顺利完成PCNL手术,试验组的结石清除率显著高于参照组,试验组的并发症发生率显著低于参照组,两组对比差异有统计学意义(P<0.05);两组的手术时长、住院时间、肾造瘘管拔除时间等指标对比,差异无统计学意义(P>0.05)。结论:复杂性肾结石采取单通道PCNL治疗时,其结石清除率低于双通道PCNL,通常还需要进行二期取石手术,而采取双通道PCNL治疗复杂性肾结石,结石清除率高,但引发并发症少,值得推广。

Objective:To investigate the effect and complications of single-channel and double-channel percutaneous nephrolithotripsy(PCNL)in the treatment of complex renal calculi.Method:From January 2016 to January 2020,60 patients with complex renal calculi were selected and randomly divided into the reference group and the experimental group,30 cases in each group.The reference group was given single-channel PCNL treatment,the experimental group was given double-channel PCNL treatment,and the treatment effect of the two groups was compared.Result:The two groups were successfully completed PCNL operation,the stone clearance rate of the experimental group was significantly higher than that of the reference group,the incidence of complications in the experimental group was significantly lower than that in the reference group,the differences between the two groups were statistically significant(P<0.05).There was no significant difference in the operation time,hospital stay,nephrostomy tube removal time between the two groups(P>0.05).Conclusion:The stone clearance rate of single-channel PCNL treatment is lower than that of double-channel PCNL,and secondary lithotomy is usually needed.However,doubli-channel PCNL treatment of complex renal calculi has high stone clearance rate,but less complications,which is worthy of promotion.
