不同剂量右美托咪定复合罗哌卡因骶管阻滞在小儿尿道下裂矫正术中的镇痛效果比较

Comparison of Analgesic Effect of Different Doses of Dexmedetomidine Combined with Ropivacaine Sacral Block in Children with Hypospadias Correction

目的:探讨不同剂量右美托咪定复合罗哌卡因骶管阻滞在小儿尿道下裂矫正术中的镇痛效果。方法:对笔者所在医院收治的92例实施尿道下裂矫正术患儿进行观察,全部患儿均实施右美托咪定复合0.2%罗哌卡因骶管阻滞,采用随机数表法将全部患者分为两组,各46例,对照组右美托咪定用量为0.5μg/ml,观察组为0.7μg/ml,比较两组患儿苏醒时间、有效镇痛时间、苏醒后不同时间疼痛程度及不良反应发生状况。结果:观察组苏醒时间和有效镇痛时间均长于对照组,差异均有统计学意义(P<0.05);观察组苏醒时,苏醒后2、6、12、24 h VAS评分均低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率对比差异无统计学意义(P>0.05)。结论:在尿道下裂矫正术患儿骶管阻滞中,相比于0.5μg/ml右美托咪定+0.2%罗哌卡因骶管麻醉使用剂量,采用0.7μg/ml右美托咪定+0.2%罗哌卡因骶管阻滞镇痛效果明显,且不良反应发生率无明显上升,同样具有一定安全性,具有推广价值。

Objective:To investigate the analgesic effect of different doses of Dexmedetomidine combined with Ropivacaine in children’s hypospadias correction.Method:A total of 92 children with hypospadias correction in our hospital were observed,all of them were treated with Dexmedetomidine combined with 0.2%Ropivacaine,and were divided into two groups by random number table method,with 46 cases each.The dose of Dexmedetomidine was 0.5μg/ml in the control group and 0.7μg/ml in the observation group.The awake time,effective analgesic time,pain degree of different times after waking up,and the occurrence of adverse reactions were compared between the two groups.Result:The waking time and effective analgesia time in the observation group were both longer than those in the control group,the differences were statistically significant(P<0.05).The VAS scores in the observation group were all lower than those in the control group when woke up and at 2,6,12 and 24 h after awakening,the differences were statistically significant(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compared with 0.5μg/ml Dexmedetomidine+0.2%Ropivacaine,the effect of0.7μg/ml Dexmedetomidine+0.2%Ropivacaine in the sacral block of children with hypospadias correction is obvious,and the adverse reactions are not significantly increased.The same kind of sacrum block has certain safety,and has the value of popularization.
